Background:Intertrochanteric fracture is one of the most common hip fractures in the clinic.It occurs in the elderly and is mainly related to the fall of the elderly and osteoporosis.Nowadays,most orthopedic surgeons advocate surgical treatment.With the development of medical technology,orthopedic surgeons start from different influencing factors and try to avoid complications and deformities to the utmost extent.However,there has been no consensus on the timing of surgery at home and abroad.This study seeks to start with clinical cases and explore the relationship between the timing of surgery and the efficacy of PFNA.Objective:Through the investigation of the relationship between different surgical timings and postoperative outcomes in patients with senile intertrochanteric fractures treated with PFNA,the effect of the timing of surgery on the postoperative outcome of intertrochanteric fractures in the elderly was sought to reveal the internal Regularity provides a basis for clinical treatment.Methods:Through the data clinic of Dongzhimen Hospital,patients with femoral intertrochanteric fractures from January 2015 to February 2017 were selected.Then,through the ward resident management platform,the PFNA procedure in the orthopedics of Dongzhimen Hospital was selected.Patients with intertrochanteric fractures in the elderly;through the ward resident management platform of the Third Affiliated Hospital of Beijing University of Chinese Medicine,the General Hospital of the People's Republic of China,and the Air Force General Hospital,after inquiry,select the application of PFNA that meets the inclusion criteria.Patients with intertrochanteric fractures in the elderly.After collecting the cases,the general data of the patients were collected:the patient's name,gender,age,occupation,home address,contact information,chief complaint,current medical history,engraving,past history,timing of surgery,postoperative follow-up of 1 year,postoperative collection 1 Data on the incidence of complications during the year,survival rate within 1 year after surgery,and Harris score of hip function 1 year after surgery.All the investigations and follow-up results were sorted and numbered.After checking,all the collected case data were input into the excel form.The time between the fracture and the operation was less than 5 days,and the time was between the fractures and the time between the operations.The group was divided into group B for more than or equal to 5 days,and the relationship between the timing of surgery and the incidence of complications within 1 year after surgery and the survival rate within 1 year after surgery were studied.Then,the groups in groups A and B were grouped according to age,and the relationship between the age at the same operation time and the survival rate within 1 year after surgery was studied.The SPSS20.0 software package was used for statistical analysis.The general data was collected by frequency statistics.The measurement data were expressed by meaną standard deviation.The count data was used to indicate the rate of complications after 1 year between the two groups.The comparison of the mortality rate after the year and the excellent rate of the hip function score after one year were all tested by the independent sample chi-square test.After the test,P<0.05 indicates statistical difference,P<0.01 indicates significant statistical difference,P>0.05 indicates no statistical difference.Results:After collecting the required data from 62 patients,the SPSS20.0 system was used to analyze the results.For elderly patients with intertrochanteric fractures,after PFNA surgery,the different surgical timings were selected for the occurrence of complications within 1 year.Significant influence,using chi-square test,P>0.05,no statistical difference.In the comparison of the mortality rate after 1 year,the chi-square test,P<0.05,there was a statistical difference.However,the comparison of the excellent rate of hip function was P>0.05,and there was no statistical difference.In the group A with less than 5 days of surgery,the patient group less than 80 years old and the group of patients older than 80 years were compared with the mortality rate after 1 year.After the chi-square test,P>0.05,no statistics.Learn the difference.In the B group with the selected operation time greater than or equal to 5 days,the mortality rate of the patient group less than 80 years old and the group of patients 80 years old or older was 1 year later,P<0.05,statistically significant,greater than or equal to The mortality rate after 1 year in the 80-year-old patient group was significantly higher than that in the 80-year-old patient group.Conclusion:Through this study,we found that patients with senile intertrochanteric fractures were treated with PFNA.The earlier the operation,the lower the mortality rate after surgery,which is beneficial to improve the quality of life after surgery.The prolongation of the disease will gradually increase,but in the complication and improvement of hip function,the timing of surgery is not a decisive factor,will not reduce the occurrence of po stoperative complications,and will not improve hip function.Early surgery will also reduce the effort,time and cost of the patient's family.For elderly patients,the sooner the surgery is performed,the better the patient will be treated as soon as possible,and the mortality rate will be greatly reduced after 1 year,but the family members should pay more attention to postoperative home care.
