Objective:To compare dynamic hip screw (dynamic hip screw, DHS) and the anti-rotation of proximal femoral nail (proximal femoral nail antirotation, PFNA) elderly patients with intertrochanteric fracture treatment data and treatment outcomes of two types of treatment of intertrochanteric fractures of elderly patients the advantages and disadvantages for the clinical treatment of femoral intertrochanteric fracture fixation provide the basis for the choice of methods.Methods : Affiliated Hospital of Shandong University of Traditional Chinese Medicine Department of Orthopaedics since March 2009 to September 2010 period were treated with complete medical records of 48 patients with DHS and PFNA treatment of intertrochanteric fractures of elderly cases were studied. 28 cases with DHS fixation, 20 patients with PFNA fixation, two were from the incision length, operation time, blood loss, postoperative ambulation time, healing time of fracture, the degree of functional recovery after several aspects were compared.Results:There were a total of 48 patients were followed up for 7 to 24 months until the fracture healed and no deaths. DHS group and PFNA group incision length, operative time, blood loss, fracture healing time was statistically significant difference in terms of (P <0.01), PFNA group compared to the DHS group, a short incision, operation time short, less blood loss, fracture healing time is short. But during the actual surgery and surgery PFNA group of patients exposed to x-ray was more than the number of DHS group. Surgical treatment of functional recovery of hip function in accordance with Harris scoring rate, reach a conclusion. DHS group was 88% excellent, 90% higher than PFNA lower group. PFNA DHS group and between groups in postoperative functional recovery of the difference was not statistically significant (P> 0.05). Conclusion:intertrochanteric fractures in the elderly in the treatment, PFNA fixed manner incision length, operative time, blood loss, fracture healing time in these areas is better than DHS fixation, but both functional recovery after area or less. Comprehensive view, PFNA tissue fixation on trauma patients, blood loss, less reliable fixation is the treatment of intertrochanteric fracture of femur in the elderly than ideal and can consider the internal fixation.
