Observation On The Clinical Effect Of Fuyuan Huoxue Decoction In Treating Hidden Blood Loss After Bio-type Total Hip Implantation With Qi Stagnation And Blood Stasis Syndrome

Objective: To study the effect of Fuyuanhuoxue Tang on the occult blood loss,dominant blood loss,hemoglobin(HGB),hematocrit(HCT),D-dimer of hip joint after biotype total hip arthroplasty,and to evaluate the clinical efficacy and safety of Fuyuan Huoxue Decoction in the treatment of occult blood loss after biotype total hip arthroplasty.Methods: 80 patients with unilateral femoral neck fracture who met the inclusion criteria were selected from the orthopaedic department of Hunan Direct Hospital of Traditional Chinese Medicine from December2017 to December 2018 and underwent total hip arthroplasty.They were randomly divided into treatment group and control group with 40 cases in each group.Two groups of patients were given intraspinal anesthesia and operated by the same group of doctors,using the posterolateral approach.The treatment group was given Fuyuan Huoxue Decoction orally for basic treatment after operation,while the control group was given Fuyuan Huoxue Decoction 1/10 dilution for basic treatment after operation.Hemoglobin(HGB),hematocrit(HCT)and D-dimer were recorded 1 day before operation,3 day,7 days and 14 days after operation in two groups.Theblood volume and drainage volume were recorded 48 hours after operation.The occult blood loss was calculated according to Gross linear equation.The clinical efficacy and changes of the two groups were compared.All data were analyzed by SPSS25.0 statistical software.Results: There was no significant difference in preoperative hemoglobin(HGB),hematocrit(HCT)and D-dimer between the two groups(P > 0.05);hemoglobin(HGB)and hematocrit(HCT)in the treatment group were better than those in the control group at 3,7 and 14 days after operation(P < 0.05);the hidden blood loss in the treatment group was significantly lower than that in the control group at 3,7 and 14 days after operation(P < 0.05).Significant difference was found between the two groups(P < 0.05).There was no significant difference in the apparent blood loss after operation between the two groups(P > 0.05).There was no significant difference in D-dimer between the two groups on 3 days after operation(P > 0.05).The D-dimer treatment group was significantly lower than the control group on 7 and 14 days after operation(P < 0.05),and the difference was significant(P < 0.05).Conclusion: Fuyuan Huoxue Decoction can reduce the loss of hemoglobin(HGB),the decrease of hematocrit(HCT)and the hidden blood loss of patients after total hip arthroplasty,so it can effectively prevent and treat the hidden blood loss after total hip arthroplasty,but it has no obviouseffect on the dominant blood loss after total hip arthroplasty.Postoperative D-dimer can reduce the risk of deep vein thrombosis(DVT)in lower extremities after total hip arthroplasty.
