ObjectivesThe inflammatory response is the pathophysiological reaction of diseases in clinical patients such as trauma,burns,shock,infection,etc.It is an important link and pathological basis of various diseases.In recent years,it has been found that the cholinergic anti-inflammatory pathway regulates the systemic inflammatory response,can significantly and rapidly inhibit the body's inflammatory response,and activates the cholinergic anti-inflammatory pathway by electrically stimulating the vagus nerve.The purpose of this study is to:1.To investigate the effects of electroacupuncture at Hegu and Quchi on the changes of body temperature in patients with community-acquired pneumonia by affecting the cholinergic anti-inflammatory pathway.2.To observe the safety of electroacupuncture at Hegu and Quchi points in patients with community-acquired pneumonia fever.3.When the immune system mediates the inflammatory response,the body produces pro-inflammatory mediators and anti-inflammatory mediators.In this study,the changes of the content of inflammatory mediators and acetylcholine transferase before and after treatment were investigated to explore the chol:inergic anti-inflammatory pathways.influences.MethodsThis study used a randomized,blinded,controlled,small-scale prospective study of adults with acquired community-acquired pneumonia.On the basis of the same anti-infective treatment,the observation group was given bilateral Hegu and Quchi.Acupuncture electroacupuncture treatment,comparing the changes of body temperature in the two groups after intervention,the changes of serum neurotransmitters,proinflammatory mediators and anti-inflammatory mediators in the vagus nerve anti-inflammatory pathway,in order to evaluate the antipyretic effect of the intervention,and explore the intervention The effect of the means on the anti-inflammatory pathway of the human vagus nerve.Grouping method:This study was conducted from September 1,2017 to February 21,2019,according to the inclusion and exclusion criteria set by the research program,from the emergency department of Guang'anmen Hospital of China Academy of Chinese Medical Sciences,community-acquired pneumonia fever patients 30 For example,30 patients were divided into a test group and a control group according to a random number table method.Treatment plan:30 patients from the Guang'anmen Hospital of China Academy of Chinese Medical Sciences from September 1,2017 to February 21,2019,who met the inclusion criteria in the emergency department,were randomly divided into the basic treatment group and the basic treatment+electroacupuncture group.The clinical information collection of 15 groups in each group was carried out,and the CRF table was designed.The pre-and post-treatment sub-body temperature,body temperature change trend,acetylcholine transferase content and pro-inflammatory/anti-inflammatory media content were collected and recorded.Analysis:The body temperature change trend,acetylcholine transferase content,pro-inflammatory/anti-inflammatory mediator content in the basic treatment group and the basic treatment+electroacupuncture group were compared.The data of the study were statistically analyzed by SPSS22.0.The normal distribution test is performed on the measurement data:the data of the normal distribution is represented by the mean plus or minus standard deviation(Meant SD),and the correlation statistical analysis uses the paired sample t test or the independent sample t test.The test standard was defined as 0.05,that is,the P value ? 0.05 was considered to be statistically significant.Results1.In the control group,the body temperature decreased by 0.2±0.09? in 30 min,and the body temperature in the experimental group decreased by 0.5±0.12? in 30min.In the experimental group,the body temperature decreased by 1.9±0.76? in 4 hours,and the body temperature in the control group decreased by 1.51±0.48? in 4 hours.The difference was statistically significant(P<0.001).At 4 hours after treatment,the pro-inflammatory indexes such as TNF-?,IL-1? and IL-8 decreased in both groups(P<0.05).The results showed that the TNF-a decreased by 14.32±5.27 pg/in the experimental group.In ml,the decrease of TNF-a in the control group was 9.2±3.36 pg/ml,the decrease of IL-1? in the experimental group was 19.74±9.23 pg/ml,and the decrease in IL-1? in the control group was 8.84±3.30 pg/ml.The decrease of-8 was 7.24±5.52 pg/ml,and the decrease of IL-1? of the control group was 4.02±2.67 pg/ml.The decrease of the three indexes in the experimental group was larger than that of the control group,and there was statistical difference(P<0.05)..4.At 4 hours after treatment,the anti-inflammatory index IL-10 of both groups increased(P<0.05).The results showed that the increase of IL-10 in the experimental group was 6.79±1.48 pg/ml,and the increase of IL-10 in the control group.For the 3.05±1.17pg/ml,the increase of the index in the experimental group was larger than that of the control group,and there was statistical difference(P<0.05).The increase of serum ChAT level in the experimental group was 37.29±9.67 pg/ml,and the control group was serum ChAT.The level of increase was 8.12±2.55 pg/ml.The increase of serum ChAT level in the experimental group was greater than that in the control group(P<0.05),and the test group increased more than the control group.Significant(P<0.05).5.During the whole treatment period,one patient in the test group showed slight dizziness when the needle was taken,and returned to normal after a few minutes of rest.Other patients did not show significant adverse reactions.Conclusions1.According to the observation of this subject,Hegu acupoint combined with Quchi acupuncture treatment can reduce the body temperature of patients with clinically acquired pneumonia fever,reduce inflammatory reaction and improve immune function.2.The mechanism of electroacupuncture treatment of Hegu acupoint combined with Quchi acupoint may be related to the stimulation of vagus nerve to activate cholinergic anti-inflammatory pathway and reduce the level of inflammatory response.However,as this topic is a small-scale exploratory study,the specific mechanism needs more in the future.Research interpretation.3.Acupuncture Hegu Quchi in this study is a common clinical antipyretic method commonly used in traditional Chinese medicine,and no serious adverse events have occurred.
