Background:More and more studies have shown that hyperuricemia(HUA)is associated with atrial fibrillation(AF).However,hypouricemia may also cause cardiovascular diseases such as AF.The aim of our study was to investigate the relationship between serum uric acid(SUA)and the occurrence of AF and left atrial enlargement(LAE).Methods:We enrolled 544 patients hospitalized in the Department of Cardiology of Beijing Hospital from June 2012 to June 2017,including 272 patients with non-valvular AF(AF group)and 272 cases of sinus rhythm(control group).We analyzed the risk factors for the occurrence of AF and the rehospitalization of AF patients.In addition,all subjects were stratified according to SUA level,illustrating the relationship between SUA and AF prevalence and LAE.Results:The level of SUA in the AF group was significantly higher than that of the control group(P<0.001).Logistic regression analysis was performed for the two groups by correcting other clinical covariates such as age,previous history of gout or hyperuricemia,history of heart failure,LVEF,LAE,LVEDD,IVST,CRE,showing that SUA level was correlated with AF(OR=1.003,P=0.021).And the history of heart failure,LVEF,LA expansion and CRE were risk factors for AF occurrence.The relationship between the above factors and the rehospitalization of patients with AF was illustrated by multiple Logistic regression analysis.We found that SUA was related to the hospitalization(OR=1.003,P=0.034)and rehospitalization(OR=1.005,P=0.011).All the subjects were divided into 3 groups according to the level of SUA(<265 umol/I,?265 and<315 umol/l,and ?315 umol/1).In the group with SUA 265-315umol/l,the AF prevalence rate and proportion of LAE patients were the lowest.With the increase or decrease of SUA,the AF prevalence and the percentage of LAE increased correspondingly,showing the J-curve phenomenon.We first reported the J-curve with statistical significance for SUA and AF.Although the J-curve between SUA and LAE was not statistically significant,it was the first time we know that J-curve appears between SUA and LAE.Conclusion:HUA is an independent risk factor for the occurrence of AF and the rehospitalization of AF patients.There is a J-curve between SUA and AF.There is a J-curve trend in SUA and LAE.It is speculated that SUA can lead to AF by affecting LA.
