Research Abstract:ovarian endometrioid carcinoma is an extraordinarily malignant epithelial ovarian cancer,the incidence of which accounts for 10%-15% of ovarian cancer.Most of the OEC occurred in perimenopause,the clinical symptoms were irregular vaginal bleeding,abdominal pain and abdominal distension,the masses of abdomen could be touched,most of the were unilateral,and the diameter of the tumors was lager.OEC presented at earlier stage(FIGO I/II),the prognosis of OEC are better than SC.Most of the studies on OEC are limited to the diagnosis and imaging of cases,but lack of knowledge and research on its clinical features and prognosis.Research Purpose: To explore the clinical characteristics of ovarian endometrioid caicinoma.Research Methods: The study includes patients who underwent the opearation treatment and were diagnosed as ovarian endometrioid carcinomas by pathology between 2002 and 2019 at the first Affiliated Hospital of Dalian Medical University.According to the inclusion exclusion criteria,20 patients with OEC were screened for eligibility,and the of the same period ovarian serous adenocarcinoma was divided into two groups: High-grade serous carcinoma(HGSC)and Low-grade serous carcinoma(LGSC).The patients' age,clinical symptoms,whether to combine endometriosis,carbohydrate antigen 125(CA125)? mass diameter?ascites and clinical stages were searched by searching the electronic medical records.The prognosis and survival status of patients with outpatient and telephone follow-up.SPSS20.0 statistical software was used to analyze the data.Result: 1.Age: The average age of OEC patients were 51.20±11.83 years old,the HGSC were 59.47 ±10.24 years old,while LGSC were 56.43 ±13.85 years old.The onset age of OEC group was statistically significant compared with HGSC group(P = 0.001).So the onset age of OEC significantly lower than that HGSC.2.Clinical symptom: 2.1 The OEC group had 20% found the disease were due to physical examination.15.2% of HGSC group;the LGSC group had 27.5%.There was no significant difference between OEC and HGSC?OEC and LGSC(P=0.818?P=0.527).The physical examination in OEC showed that the disease was similar to HGSC and LGSC.2.2 There were 35% of vaginal bleeding in OEC group,8.9% in HGSC group and 5% in LGSC group.There was significant difference between OEC and HGSC?OEC and LGSC(P=0.002?P=0.007).There was no significant difference between HGSC and HGSC(P=0.634).The first symptom of OEC group was abnormal vaginal bleeding than that of HGSC and LGSC group.2.3 OEC group complained of abdominal distention in 40%;61.4% of HGSC group;The LGSC group had 27.5%.There were no significant difference between OEC and HGSC?OEC and LGSC(P=0.067?P=0.201).The first symptom of OEC was that the abdominal distention was similar to HGSC and LGSC.2.4 25% of OEC group conscious abdominal mass;8.9% of HGSC group;The LGSC group had 15%.There were no significant difference between OEC and HGSC?OEC and LGSC(P=0.069?P=0.555).The first symptom of OEC was that conscious abdominal mass was similar to HGSC and LGSC.3.10% of OEC merged with EMs;2.5% HGSC merger EMs;There are no integrated EMs patients in LGSC.There was no significant difference between OEC and HGSC?OEC and LGSC(P=0.137?P=0.107).The OEC combined EMs were no significantly higher than HGSC and LGSC.4.70% of OEC patients were more than 10 cm in mass diameter;41.1% of HGSC group;The LGSC group had 42.5%.There was significant difference between OEC and HGSC?OEC and LGSC(P=0.014?P=0.044).There was no significant difference between HGSC and HGSC(P=0.876).OEC was significantly reduced compared with HGSC and LGSC.OEC group was larger than that of HGSC?LGSC group.HGSC group and LGSC group had similar tumor diameter. 80% of OEC patients with tumor located on one side,20% of patients with tumor in bilateral side;36.7% of HGSC patient tumor in one side,63.3% of patients tumor in bilateral side;47.5% of LGSC patients tumor in one side,52.5% of patients tumor in bilateral side.There were significant differences between OEC and HGSC and LGSC(P=0?P=0.016).The mass in OEC group were mostly located on one side,and those in HGSC group and LGSC group were bilateral.5.25% of OEC patients with CA125 level ?35U/ml,30% patients were between 35-200U/ml,30% patients were between 200-1000U/ml,15% patients were ?1000U/ml;6.3% of HGSC patients with CA125 level ?35U/ml,22.2% of patients were between 35-200U/ml,39.9% patients were between 200-1000U/ml,31.6% patients ?1000U/ml;5% of LGSC patients with CA125 level ?35U/ml,30% patients were between 35-200U/ml,37.5% patients were between 200-1000U/ml,17.5% patients ?1000U/ml.The difference between OEC and HGSC was statistically significant(P=0.045).There was no significant difference between OEC and LGSC(P=0.696).OEC serum CA125 level was significantly lower than HGSC,similar to LGSC.6.15% of OEC patients had more than 1000 ml of ascites,25% patients were between 100-1000 ml,15% patients were ?100ml;31% of HGSC patients had more than 1000 ml of ascites,30.4% patients were between 100-1000 ml,38.6% patients were ?100ml;30% of LGSC patients had more than 1000 ml of ascites,20% patients were between 100-1000 ml,50% patients were ?100ml.There was no significant difference between OEC and HGSC?OEC and LGSC(P=0.156?P=0.426).The ascites of OEC was similar to HGSC and LGSC.20% of OEC patients had the positive rate of ascites cells;31.0% of HGSC group;The LGSC group had 37.5%.There were significant differences between OEC and HGSC and LGSC(P=0?P=0.002).The positive rate of ascites cells in OEC patients was significantly lower than HGSC and LGSC.7.80% of OEC patients were in the early stage(stage I+II),and 20% were in the late stage(stage III+IV).31.6% of HGSC patients were in the early stage and 68.4% in the late stage;47.5% of LGSC patients are in the early stage,52.2% in the late stage.The differences between OEC and HGSC,OEC and LGSC were statistically significant(P=0.0,P=0.016).Compared with HGSC and LGSC,the proportion of early patients in OEC increased significantly.8.The 3-year overall survival rate of OEC was 87.5%,the 3-year overall survival rate of HGSC was 76.4%,and the 3-year overall survival rate of LGSC was 68.1%,which was not statistically significant compared with OEC(P=0.658,P=0.669).The average survival time of OEC was 79 months,the average survival time of HGSC was 100 months,and the average survival time of LGSC was 99 months,which was not statistically significant compared with OEC(P=0.822,P=0.93).OEC has a similar prognosis to SC.Conclusion: The clinical features of OEC are different from SC:1.The average age of OEC patients were 51.20 years old,the onset age of OEC significantly lower than that HGSC,the onset age of OEC similar to LGSC.2.The incidence of abnormal vaginal bleeding in OEC was significantly higher than SC.3.Most of the OEC tumors were unilateral,and the diameter of the tumor was larger than HGSC,most of the SC tumors were bilateral.4.OEC serum CA125 level was significantly lower than HGSC,and the pathological positive rate of the ascites cells was significantly lower than SC.5.There were significantly more early cases of OEC than SC?...
