Research On The Service Model Of Integrated Medical Care For The Elderly With Chronic Diseases In Urban Areas From The Perspective Of SWOT Analysis

At the two sessions of the 2018 national conference,the premier of the State Council Li Keqiang pointed out in the government work report: "actively dealing with the aging of the population,developing homes,communities and mutual support for the elderly,promoting the combination of medical treatment and endowment and improving the quality of service in nursing homes." In the meantime,“endowment” is also one of the most popular key words in the people's livelihood field of the two sessions this year,with the frequent appearance in the bills,proposals and suggestions of the members of the Committee.With the acceleration of population aging in China,the chronic disease has become an important problem affecting the quality of life of the elderly.The yearly increasing number of the elderly people with chronic diseases in the towns and the imperfect system of elderly care service has become a serious pension problem,which is an increasingly prominent contradiction in the current aging society.In order to better meet the needs of the urban elderly with chronic diseases in life and health care,this article uses the SWOT analysis method to innovate the ways of cares for the aged according to the needs,increasing the health care function on the basis of providing old-age services,integrating existing pension resources and medical resources,encouraging endowment institutions to establish medical services and medical institutions to develop endowment services or encourage cooperation between endowment institutions and medical institutions to improve the quality of life of the elderly.This paper mainly studies the old-age care for the elderly with chronic diseases,carries forward the concept of "old people have medical care,old people have support",so as to understand the service mode of the elderly with chronic diseases,according to the research content,analyzes the development background of the old-age care institutions in Nanchang,and finally selects several old-age care institutions in Qingshan Lake District for field investigation.Through interviewing the dean,nurses,nurses,the elderly and their relatives in the residential institutions,the paper collects the necessary information,and analyzes the advantages and disadvantages of the combination of medical care and nursing,so as to provide a favorable guarantee for the development of the paper.
