Research On The Current Status And Influencing Factors Of Family Doctor Services In Xi'an

In recent years,the trend of population aging in China is severe,and the number of patients with chronic diseases is increasing year by year.The current medical and health service system can not meet the needs of the masses for medical treatment in this context.To adapt to this change,based on summarizing the development of foreign medical and health undertakings,and combining the situation of our country,the development mode of family physician service is put forward.Family physicians the core provide agreed medical services to the contracted residents to meet their continuous long-term health care needs by signing the contract.Family physician service is an effective measure to standardize residents' orderly medical treatment,control medical expenses and alleviate the status of "expensive and difficult medical care" for residents.At present,due to the imperfect supporting policies,the shortage of general practitioners,the system of encourage is not complete,the doctor lack of service enthusiasm,the resident attitude toward contract is indifferent,there is no unified mode for the development of contracted services of family doctors.There are some problems in the process of development in many places.This article takes Xi'an community residents as the research object.The status of family physician services in three selected community residents in Xi'an is studied by using questionnaire survey,in-depth interviews and SPSS statistical analysis.The purpose of this study is to analyze the current status of family physician service in Xi'an,to identify problems in service development,and to explore solutions.References for improving the service model of family doctors,advancing medical and health system reform,and developing community health services will be provided.This article is divided into six parts:1.Introduction.This part mainly expounds the research background,the research significance,the domestic and foreign research status,the research methods,the research content and the innovation point of the rese-arch;2.Definition and theoretical basis of related concepts.This part mainly expoun-ds the"family physician","contract service","classification of treatment" and "two-way referral" concept,introduces the theory of division of labor,demand-motivation theory,incentive compatibility;3.Survey results of physician doctor service in Xi'an.This part mainly describes the overview of family physician services,the knowledge and needs of non-contracted residents on the service of family physician,the residents' knowledge and satisfaction with family doctor services and the results of interviews;4.Analysis of the influence factors of the renewal or contract of the residents.This part mainly describes the influencing factors of non-contracted residents' contracting intentions and contracted residents' willingness to renew contracts;5.Analysis of the main problems and reasons of the service in the process of implementation.The main problems are inadequate policy promotion,residents' low signing intentions and family physicians'inactive services;6.Countermeasures and suggestions.Strengthening the propaganda of policy,enhancing the residents' awareness of prevention and health care,improving the income of family physicians,formulating performance evaluation and incentive system,strengthening the guiding role of medical insurance are put forward.The main conclusions of this article include:At present,the number of contracts for family doctors is considerable but the quality is worried;Residents lack of understanding of services and there is a crisis of trust for family doctors;Young residents have poor awareness of preventive care,their view of medical has not changed;Residents' willingness to sign is not high,and influenced by many factors;The number of general practitioners is short,and the gap between service ability and residents' expectations is exist;The family doctor's service enthusiasm is not high,because of incentive system is not complete.The innovative view of this article is selects Xi'an community residents as the research object,Expanded the research area,and enriched research content.
