The Intervention Study Of Image Dialogue On Depressive Symptoms Of Renal Dialysis Patients

Depression is the most common mental disorder in MHD(maintain hematodialysis)patients.It is an independent risk factor that affects the prognosis.Effective psychotherapy can enhance the immunity of MHD patients and make them in good condition for treatments.As the symptoms of depression improve,so does the effect of dialysis.In order to understand the morbidity and influence factors of MHD patients' depression,Self-rating depression scale(SDS)and compiled questionnaire of general situation were adopted to survey 309 patients from two Tertiary Hospitals of Yangzhou city.184 MHD patients combined with depression were identified and compared to the 198 depressed person without physical disease.The traits of MHD combined depression were explored.In order to observe the curative effect of imagery communication psychotherapy on MHD depression,30 subjects were selected randomly from the 184 patients and divided into an intervention group and a control group equally.The intervention group was performed once a week,with 12 sessions of psychotherapy.The results of the study were as follow:1.The incidence of HMD depression is 59.5%,out of the 184 MHD depression patients.103 were mild depression,72 were medium depression,9 were major depression.2.In the total score and each dimension score of SDS,there were no significant differences between the MHD depression patients and depressed person.But the typical depressive symptoms were different.3.A positive correlation was found between the degree of depression and duration of MHD and a negative correlation were found between the degree of depression and the nutritional status.4.Both the HMD depression patients and the depressed person had some same imagery traits,they are desolations,loneliness,lack of motivation.On this basis,there was more imagery about "dirty water" of MHD depression patients.5.After the psychotherapy to the intervention group,the SDS score had a dramatic decline and the symptoms of depression were significantly reduced.The indicators about dialysis effect,such as C-reactive protein,serum albumin and Creatinine clearance rate were all better than the control group.This study shows that Imagery Communication Psychotherapy has an exact effect on MHD depression.It is easy to operate and has no burden on patients.It is worth popularizing in clinic.
