A Study On Improving The Social Adaptation Ability Of Severely Disabled Persons By Case Management

Severe disabled people are social vulnerable groups.The problems they face are multiple and complex.At this stage,the government's assistance to severe disabled people mostly stays at the level of material assistance,far from meeting the needs of severe disabled people.For this group,the social living environment is very unsatisfactory.The author hopes to use case management method to provide resource links in employment,education and medical care for the severely disabled so as to help them tap their own potential,enhance their ability to solve problems independently,improve family relations and improve social support networks,so as to enhance the social adaptability of the severely disabled and promote their better integration into society.The author takes Zhang Mou,a severely disabled person in Shangjie District of Zhengzhou City,Henan Province,as the object of service,guided by the theory of social support network and the theory of energy enhancement,and provides services for the case owners by the method of case management.By contacting the case owner and his family,the author understands the basic information of the case owner,clearly defines the problem of the case owner,links employment resources and psychotherapy resources for the case owner,helps them improve family relations and build social support network,and finally enhances the case sovereignty.Through the process evaluation and result evaluation,the author completed the plan of each stage,reached the goal of case management service.After the end of the service relationship,the author also provided regular follow-up service for the case owners to ensure that the service effect can be sustained,and finally the case owners' living conditions have been improved.Based on the practice of reflection and summary,the author believes that case management method is effective in the service of severely disabled people,and puts forward some corresponding suggestions,hoping to be helpful for the development of social work.
