| Objective: Panic Disorder(PD)is an acute anxiety disorder characterized by recurrent unpredictable panic attacks,often accompanied by sweating,trembling,palpitations,dizziness or chest pain,dead feeling,loss of control and so on.The patient continues to worry about future panic attacks or their consequences,with avoidance of certain places in the environment.Patients frequently see doctors because of recurrent severe physical symptoms,seriously affecting the quality of life of patients and social functions and causing a high individual,social and economic burden.At present,a larger number of literatures have proposed biological,cognitive and behavioral hypothesis of panic disorder.O ne of the most influential hypotheses is the neurobiology hypothesis of panic disorder proposed by Gorman,who believes that panic disorder patients inherit an amygdala-centered fear network.Quantitative electroencephalogram(QEEG)is a very accurate measure of the current state of the brain and its changes in amplitude,frequency and topography reflect changes in brain function.In clinical studies,QEEG is a very important tool to measure the abnormal parameters of mental disorders.In the past,the conclusions of EEG study in panic disorder have not been reached yet,The relationship between EEG and the clinical manifestations is not clear.In this study,QEEG technique was used to analyze the EEG characteristics in patients with panic disorder under resting and music conditions,eyes open and closed state.we try to find the EEG power differences between the patients of panic disorder and healthy people,study whether the music condition can regulate the brain activity of panic disorder and explore the mechanism of music therapy.Methods: 25 patients with panic disorder who met the diagnostic criteria of DSM-V and 24 matched healthy controls by sex,age and educational level participated in this study.All subjects completed Hamilton Anxiety Rating Scale(HAMA),Hamilton Depression Rating Scale(HAMD),State-Trait Anxiety Questionnaire(STAI)and Eysenck Personality Q uestionnaire RSC(EPQ-RSC).In addition,panic disorder subjects were asked to complete Panic Disorder Severity Scale(PDSS)and Panic-Associate Symptom Scale(PASS)to assess the severity of disease.EEG data were recorded in the music condition(eyes-open state and eyes-closed state)and resting condition(eyes-open state and eyese-close state)for 9 minutes,respectively.We use the EEGLAB software package in MATLAB to process EEG data offline,and calculate the absolute power for 3 frequency bands: θ(4-8Hz),α(8-13Hz),β(13-30Hz).SPSS 20.0 was used for statistical analysis.The HAMA,HAMD,STAI and EPQ-RSC scores of two groups were tested by Independent-samples t-test.A repeated-measures ANOVA with Condition,State and Region within-subjects factors,and Group between subjects factor,compared the groups for α absolute power,β absolute power,frontal alpha asymmetry(FAA)and Frontal midline theta(FMT).Pearson correlation analysis was made between absolute power values of each band and HAMA,HAMD,STAI,PDSS and PASS scores under resting condition and music condition in panic disorder.Results:1.The scores of the HAMA(t = 13.94,p < 0.001)、HAMD(t = 8.76,p < 0.001)、STAI-State(t = 3.81,p < 0.001)、STAI-Trait(t = 3.32,p < 0.01)、EPQ(N)(t = 5.23,p < 0.001)between panic disorder patients and healthy controls are significantly different.2.The β power of panic disorder group was significantly higher than healthy control group(F = 4.32,p = 0.043).Pearson correlations(two-tailed)showed a significant positive correlation between β power and PDSS score in patients with panic disorder(r = 0.46,p = 0.022).3.The FMT of two groups during the music condition were significant higher than the restimg condition(F = 4.90,p = 0.032).In the eyes-open state,the α power during the music condition was higher than the resting condition in PD group(F = 4.16,p = 0.047).4.There was no significant difference in FAA between panic disorder group and healthy control group(F = 1.84,p = 0.181).Conclusions:1.The β power of patients with Panic disorder is abnormal compared with healthy people,indicating that patients have the expected anxiety in the resting state,highly sensitive to their own or outside information,their brain in alert state.2.β-power and PDSS was positively correlated in Panic disorder patients,indicating that β band has a certain correlation with the severity of panic disorder symptoms.3.Music have an regulatory role in brain activities of patients with panic disorder,It can help patients relieve anxiety and promote physical and mental relaxation by improving alpha power and FMT of patients with panic disorder. |
