OBJECTIVE: To observe the effect of enteral nutrition combined with probiotics on intestinal mucosal barrier and immune function in patients with decompensated cirrhosis.METHODS: A total of 90 patients with decompensated cirrhosis with nutritional risk were selected from Nutrition Risk Screening(NRS2002?3 points).There were 45 cases in each group.Both groups were given treatments such as protecting the liver,turning yellow,reducing enzymes,improving liver microcirculation,and dealing with complications.The observation group added oral enteral nutrition preparations in combination with probiotic nutritional support.Observe and record the changes of NRS2002 score,body mass index,liver function,intestinal mucosal barrier and immune function related indexes of the two groups of patients 1 day before nutrition,7 days after nutrition and 28 days after nutrition,and observe liver function indicators(plasma albumin,Pre-albumin,transaminase),intestinal barrier function indicators(DAO,D-lactic acid,TNF-?),cellular immunity indicators(CD3 +,CD4 +,CD4 + / CD8 +),humoral immunity indicators(IgA,IgG,IgM)Science analysis.RESULTS:(1)Human parameters and NRS2002 scores: There was no significant difference in body mass index between the observation group before nutrition,the 7th day and 28 th day after nutrition compared with the control group(P>0.05);The 28-day NRS2002 score of the observation group was lower than that of the control group,and the difference was different It was statistically significant(P<0.05).In the observation group,the NRS2002 scores decreased 7 days and 28 days after nutrition compared with 1 day before nutrition,and the difference was statistically significant(P<0.05).(2)Liver function indexes: The albumin level of the observation group on the 28 th day after nutrition was higher than that of the control group,the difference was statistically significant(P<0.05).Pre-albumin levels in the observation group were significantly different from the control group at 7 days and 28 days after nutrition(P<0.05).The levels of albumin and pre-albumin on the 7th and 28 th day after nutrition in the observation group were higher than the 1 day before nutrition,and the difference was statistically significant(P<0.05).(3)Cell immunity index: The levels of CD3 + T and CD4 + T in the observation group were higher than those in the corresponding control group on the 28 th day after nutrition,and the difference was statistically significant(P<0.05).The levels of CD3 + T and CD4 + T on the 28 th day after nutrition were significantly higher than those on the 1st day and 7th day after nutrition,and the difference was statistically significant(P<0.01).(4)humoral immune index: there was no significant difference between the observation group and the control group in serum IgA,IgM and IgG levels at all times(P>0.05);(5)DAO,D-lactic acid,TNF-?: The levels of DAO and TNF-? on the 28 th day after nutrition were lower than those of the control group,the difference was statistically significant(P<0.05);the level of DAO on the 28 th day after nutrition was lower than that before 1 day and 7 days after nutrition,the difference was statistical academic significance(P<0.05).and the level of TNF-? on the 28 th day after nutrition was lower than that on the day before nutrition,the difference was statistically significant(P<0.05).CONCLUSIONS:Enteral nutrition combined with probiotics is beneficial to reduce the NRS2002 score of patients with decompensated liver cirrhosis,improves albumin and pre-albumin levels and cellular immune function,inhibit the secretion of inflammatory factors,and have a certain effect on the repair of intestinal mucosal barrier,but in the short term Changes in body mass index and enhancement of humoral immunity in patients had no significant effect. |
