Correlation Between The Quantitative Analysis Of The Common Femoral Vein Using Ultrasound And The TCM Syndrome Of Lower Extremity Varicose Veins

Objective: By analyzing the correlation between the results of color Doppler ultrasound examination of the common femoral vein in patients with varicose veins of lower extremity and the traditional Chinese medicine(TCM)syndromes of patients,the difference of ultrasonic examination of different TCM syndromes in patients with varicose veins of lower extremity was discussed,and provides some objective indexes for the treatment of TCM syndrome differentiation.Methods: In this study,the patients with varicose veins of lower limbs were selected from January 18,2019 to July 17,2020 in the outpatient department of vascular surgery of the Second Hospital of Fuzhou,and were divided into three groups according to TCM syndrome differentiation(qi deficiency and blood stasis syndrome group,cold and dampness coagulation syndrome group,dampness-heat diffusing downward syndrome group).The common femoral vein of each person was examined by ultrasound,and the data of diameter,flow rate and reflux were recorded.Statistical analysis was carried out to investigate the correlation between different TCM syndromes of varicose veins of lower extremity and the index of the common femoral vein under ultrasound.Results: The total number of patients with varicose veins of lower extremity who met the inclusion criteria was 110 after exclusion and elimination.After TCM syndrome differentiation,there were 44 cases in qi deficiency and blood stasis syndrome group,35 cases in cold and dampness coagulation syndrome group and 31 cases in dampness-heat diffusing downward syndrome group.(1)The age,sex,course of disease,limb distribution(left and right distribution,single and bilateral distribution)of the patients in the three groups were not statistically significant(P>0.05),which was comparable.(2)There was significant difference in the diameter and velocity of the common femoral vein(0.5 cm at the midpoint of the inguinal ligament)measured under ultrasound(P<0.05).The order of pipe diameter from coarse to fine is dampness-heat diffusing downward syndrome group,qi deficiency and blood stasis syndrome group,cold and dampness coagulation syndrome group.The order of flow velocity from fast to slow is dampness-heat diffusing downward syndrome group,qi deficiency and blood stasis syndrome group,cold and dampness coagulation syndrome group.It was found that the difference of diameter and velocity of the common femoral vein was mainly between the dampness-heat diffusing downward syndrome group and the cold and dampness coagulation syndrome group.According to the reflux time of the common femoral vein(medial 0.5 cm of the middle point of the inguinal ligament)was divided into three grades in the three groups,and the difference was statistically significant(P<0.05).The difference was mainly between the cold and dampness coagulation syndrome group and the dampness-heat diffusing downward syndrome group,the cold and dampness coagulation syndrome group and the qi deficiency and blood stasis syndrome group.In the reflux condition,the proportion of severe reflux was classified from high to low as qi deficiency and blood stasis syndrome group,dampness-heat diffusing downward syndrome group,cold and dampness coagulation syndrome group,moderate reflux ratio from high to low as cold dampness coagulation syndrome group,dampness-heat diffusing downward syndrome group,qi deficiency and blood stasis syndrome group,the proportion of normal valve function from high to low was classified as cold and dampness coagulation syndrome group,dampness-heat diffusing downward syndrome group,qi deficiency and blood stasis syndrome group.Conclusion: 1.The results showed that patients with lower extremity varicose veins with thicker diameter and faster velocity of flow of the common femoral vein under ultrasound tend to dampness-heat diffusing downward syndrome,patients with lower extremity varicose veins with thin diameter and slow flow rate of the common femoral vein tend to cold and dampness coagulation syndrome group,patients with varicose veins of lower extremities with long reflux time of the common femoral vein tend to qi deficiency and blood stasis syndrome group.2.The diameter,flow rate and reflux of the common femoral vein measured under ultrasound in patients with varicose veins of lower extremities have some correlation with the different TCM syndromes.These parameters can provide some reference for TCM syndrome differentiation and treatment of patients with varicose veins of lower extremities.
