ObjectiveTo evaluate the clinical efficacy of Electro-acupuncture in the treatment of migraine and investigate the correlation amongst the Chinese Medical Constitution,Syndrome Type and the Migraine Attack in order to provide the evidence-based evidence for clinical standardized Electro-acupuncture treatment of migraine.MethodsThe patients were selected from the Acupuncture Department of Longhua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine and the Acupuncture Department of Huadong Hospital in Changning District of Shanghai from March 2017 to December 2018.The 60 patients with migraine were randomly assigned to the Electro-acupuncture treatment group and the oral drug control group with reference to the “Clinical Research Data Collection and Management System”.The control group received conventional drug treatment, using “Xibiling Capsule”(Flunarizine hydrochloride capsule),5mg,1 time per night in a total of 10 times. The treatment group received electro-acupuncture treatment, taking the acupuncture point for Bai hui(DU20),Feng Chi(GB20),Wai Guan(SJ5),He Gu(L14),Tai Chong(LR3)and A shi points. The pattern of ascendant hyperactivity of liver yang syndrome is equipped with acupuncture points,Xing Jian(LR2),He Yan(GB4)、Xuan Lu(GB5);The pattern of Phlegm-Turbidity-Flooding-Upward Syndrome using Zhong Wan(RN16)、Feng Long(ST40),San yin Jiao(SP6);The pattern of Static blood obstructing the Network,Vessels syndrome using Xue Hai(SP10)and Ge Shu(BL17);Qi4 and blood vacuity using Qi hai(RN6),Zu San Li(ST36)and Pi Shu(BL20);Liver and Kidney Yin deficiency syndrome with Shen Shu(BL23),San yin jiao(SP6),Tai Xi(K13).3 times a week and 10 times for a course of treatment. The primary outcome measures are the Visual Pain Simulation Rating Scale(VAS),At the same time, the correlation between TCM constitution and syndrome type and VAS score was statistically analyzed.the Secondary Outcome Index Headache Impact Measurement Scale(HIT-6),the Outcome Scale reported by migraine patients, and the Quality of Life Questionnaire(MSQ)for migraine patients. The time points of observation were baseline(W0),the end of 4 weeks’ treatment(W4),and follow-up after 16 weeks(W16).Results1.The scores of VAS,HIT-6,PRO and MSQ in the electro-acupuncture group and the oral drug control group significantly improved at the end of treatment(W4)compared with baseline(W0).At follow-up(W16),The above indexes in the treatment group still improved, while Those in the control group gradually increased to the pre-treatment level. The differences were statistically significant(P<0.05);2.After treatment, the reduction of scores of VAS,HIT-6,PRO and MSQ in the electroacupuncture treatment group were significantly larger than those in the oral drug control. The differences were statistically significant(P<0.05).3.There were significant differences in VAS values between every two observation time points in each TCM syndrome types(P < 0.05).Among them, the VAS values of the patients with syndrome of ascendant hyperactivity of liver yang and phlegm-turbidity disturbing mind decreased the most significantly after electro-acupuncture treatment, and there was no significant difference between the two groups before and after treatment and follow-up(P > 0.05).4.There were significant differences in VAS values between every two observation time points in each TCM constitution group(P < 0.05),among which the VAS values of patients with qi-stagnation constitution and special-nature constitution decreased the most significantly after electro-acupuncture treatment, and there was no significant difference in VAS values between groups before and after treatment and during follow-up(P > 0.05).5.There was no adverse reaction of acupuncture during treatment.Conclusion1.Electro-acupuncture can relieve pain degree of migraine patients, and has long-term effect and good safety,2.There was a more effective trend in electro-acupuncture on migraine in syndromes of hyperactivity of ascendant hyperactivity of liver yang and phlegm-turbidity disturbing mind.3.There was a more effective trend in electro-acupuncture on migraine in patients with qistagnation constitution and special-nature constitution.
