Based On Data Mining Professor Zhou Yongming Treatment Of Chronic Aplastic Anemia Drug Law Research And Clinical Observation Of Empirical Prescription

Objective1.To explore the medication rule of Professor Zhou Yongming in treating chronic aplastic anemia.2.To observe the clinical effect of Jianpi Bushen empirical prescription based on data mining technology on chronic aplastic anemia.3.To explore the possible therapeutic mechanism of Jianpi Bushen empirical prescription for chronic aplastic anemia.Methods.1.Collect,collate and summarize the CAA outpatient medical records.60 cases of 318 cases with definite diagnosis and relatively complete records are pre-processed.Professor Zhou Yongming's CAA medical records information system is constructed by using the clinical collection system in the software of Chinese Medicine Inheritance Assistant Platform(V2.5).Frequency analysis is carried out under the modules of prescription management,statistical reports and prescription analysis.Data mining is carried out by meridian regression analysis and systematic cluster analysis.Professor Zhou's medication rules and characteristics are summarized from the aspects of classification of traditional Chinese medicine,meridian regression of high-frequency drugs,prescriptions of high-frequency classification and new clustering.The statistical analysis results are combined with the actual clinical application to form an empirical prescription for invigorating the spleen and invigorating the kidney,which provides a new idea for clinical treatment.2.45 patients with CAA and 10 healthy persons were selected.The levels of vascular endothelial growth factor and HIF-1a in peripheral blood were detected by ELLSA.The differences between the two groups were analyzed to explore the pathogenesis of CAA.3.45 patients with CAA were randomly divided into treatment group(30 cases)and control group(15 cases).The control group was treated with routine western medicine,while the treatment group was treated with experience prescription of invigorating spleen and invigorating kidney based on the control group.The total effective rate of treatment was analyzed and compared between the two groups.The differences of the degree of bleeding,TCM symptom score,blood routine,vascular endothelial growth factor and HIF-1a before and after treatment were explored to explore the difference of invigorating kidney and invigorating spleen The efficacy and mechanism of the prescription.Results1.Professor Zhou Yongming treated CAA with 134 kinds of traditional Chinese medicines.The commonly used single medicines were Psoralea corylifolia,Ligustrum lucidum,Eucommia ulmoides,Radix Polygoni Multiflori,Salvia Miltiorrhiza,Peony Peel,Fried Atractylodes macrocephala,Epimedium,Fried Paeonia lactiflori,Herba Hedyotis dii,Astragali,Cuscuta seed,Fried Licorice Root,Selaginella,Lycium barbarbarum,Radix Selaginellae,Radix Pseudostellariae,Rhizoma Pinelliae,Chinese Yam,Radix Rub Zi et al.The commonly used Chinese medicine types are tonifying deficiency drugs,promoting blood circulation and stopping bleeding,clearing away heat drugs,etc.According to the classification of drug meridians,they are liver,spleen,kidney,lung,stomach,heart,small intestine,large intestine,gallbladder and pericardium.Five core drug groups were obtained by cluster analysis.The first group was Astragalus membranaceus,Cuscuta chinensis,Ligustrum lucidum,Atractylodes macrocephala and Paeonia lactiflora.The second group was Bombyx mori,Steamed bun,Geranium wilfordii and Paeonia suffruticosa.The third group was dandelion,Scutellaria baicalensis,trichosanthin,chrysanthemum and lentil.The fourth group was Salvia miltiorrhiza,Sedum notoginseng,Hawthorn and Selaginella.The fifth group was Epimedium,Psoralea,Yam,Radix Pseudostellariae,Ligustrum Lucidum,Radix Polygoni Multiflori and Achyranthes bidentata.2.Compared with the normal control group,the levels of vascular endothelial growth factor and HIF-1a in CAA group were significantly lower than those in normal control group(P<0.05).There was no significant difference between HIF-1a and normal control group(P>0.05).There were significant differences in the phenotypic proportions of vascular endothelial growth factor and HIF-1a between CAA group and normal control group(P < 0.05).There was a slight correlation between the phenotypic proportions of vascular endothelial growth factor and HIF-1a(r = 0.368).3.Comparisons between CAA treatment group and CAA control group before and after treatment:(1)Blood routine: There was no significant difference in platelet count between CAA treatment group and CAA control group before and after treatment(P > 0.05).After treatment,the white blood cell count increased from(2.80 +1.10)to(3.70 +0.80),the hemoglobin concentration increased from(76.50±28.98)to(101.50±22.41),and the neutrophil count increased from(1.00 +0.80)to(1.20 +1.60).There were significant differences(P <0.05).There were significant differences in white blood cell count,hemoglobin concentration and neutrophil count before and after treatment in CAA control group(P>0.05).After treatment,the hemoglobin concentration increased from(52.00 +36.00)to(82.00 +71.25),and there were significant differences before and after treatment(P<0.05).(2)Comparison of the levels of vascular endothelial growth factor and HIF-1a: After treatment,the levels of vascular endothelial growth factor and HIF-1a in CAA treatment group were higher than those before treatment(P<0.05),and there were significant differences in the levels of vascular endothelial growth factor before and after treatment in CAA control group(P<0.05),while there were no differences in the levels of HIF-1a before and after treatment(P > 0.05).Conclusion1.Professor Zhou Yongming's basic principles in treating CAA are strengthening the spleen and kidney to consolidate the root,reducing fire and stopping bleeding to cure the symptoms,activating blood circulation and removing blood stasis to regenerate the new,and adopting various methods to achieve success.The main treatment methods are invigorating the spleen and kidney,promoting blood circulation and stopping bleeding,etc.Through data mining,we can draw Professor Zhou Yongming's medication rules for treating CAA,which can provide quantitative data for better inheriting his academic ideas and clinical experience It is supported.2.The experience prescription of invigorating the spleen and invigorating the kidney combined with androgen in the treatment of CAA can improve the clinical symptoms and relieve the bleeding symptoms,at the same time,improve the peripheral blood picture and the levels of vascular endothelial growth factor and HIF-1a,which are obviously better than the control group with androgen alone.It will provide theoretical and experimental basis for further study of aplastic anemia and research,development and application of new medicines of experienced prescriptions of famous doctors.3.The therapeutic effect of Jianpi Bushen Experience Prescription on CAA is mainly manifested in the increase of leukocytes,hemoglobin and neutrophils.3.The mechanism of the therapeutic effect of Jianpi Bushen Recipe on CAA is related to the levels of vascular endothelial growth factor and HIF-1alpha,which can improve the levels of vascular endothelial growth factor and HIF-1alpha,and thus play a corresponding therapeutic role.
