Objective: To explore the clinical study of the relationship between TCM syndromes,Near Infrared Thermography(NIRT)and KOA symptoms in Knee Osteoarthritis(KOA).Methods: To collect 90 cases of KOA patients from January 2017 to November 2017 in the orthopaedic and traumatological clinic of Shanghai Shuguang Hospital and meet the diagnostic criteria of traditional Chinese and Western medicine for knee osteoarthritis(2015 edition),they were divided into cold-dampness obstruction type,liver-kidney deficiency type and blood stasis-heat type.TCM data were obtained through four diagnostic instruments,and WOMAC score scale(The Western Ontario and Mc Master Universities Ost Ost)was filled in.Earthritis Index(WOMAC),near infrared thermal imager captured the superficial skin temperature of five regions of interest(region of Interests,ROI)of the front patella,subpatella,medial patella,lateral patella and central patella of the knee joint,and explored the correlation between knee joint temperature,WOMAC score and TCM syndromes.Result: 1.Three groups of TCM syndromes of 90 patients,20 males and 70 females;colddampness obstruction type of 34 people(37.8%),6 males and 28 females;deficiency of liver and kidney type of 27 people(30.0%),9 males and 18 females;stagnation-heat type of 29 people(32.2%),5 males and 24 females.2.There was statistical difference between gender and WOMAC dysfunction score(t = 2.339,p = 0.020),but there was no correlation between gender and WOMAC pain,stiffness score and total score(p > 0.05).3.There was a weak positive correlation between age and WOMAC pain score(P=0.021,r=0.243),dysfunction score(p =0.031,r=0.229)and total score(p =0.015,r=0.256),but no correlation with stiffness score(p =0.395).4.BMI was positively correlated with WOMAC pain score(p =0.007,r=0.281),dysfunction score(p =0.006,r=0.027)and total WOMAC score(p =0.004,r=0.300),but not with WOMAC stiffness score(p =0.798).5.There was statistical difference in the stiffness score of WOMAC among the three types(p < 0.000).There was no statistical difference in pain,dysfunction and total score of WOMAC(p > 0.05).6.The calorific values of five ROI points of cold-dampness obstruction type,liver-kidney deficiency type and blood stasis-heat mutual junction type were significantly different in patella(p =0.001),subpatella(p =0.001),medial patella(p =0.004),and medial patella(p =0.013).The calorific values of three types were not significantly different in lateral patella(p =0.062).7.There was a significant negative correlation between the lateral patellar calorific value of cold-dampness obstruction type and WOMAC pain score(p =0.021,r=-0.395);there was a significant negative correlation between the lateral patellar calorific value of liverkidney deficiency type and WOMAC pain score(p =0.304,r=-0.037);there was no significant difference between the five ROI calorific values of blood stasis-heat junction type knee joint and WOMAC score(p >0.05).Conclusion: 1.There are statistical differences in the calorific value of NIR imaging between patients with knee osteoarthritis of different TCM syndromes,which indicates that NIR thermal imaging has auxiliary function in the diagnosis of TCM syndromes of knee osteoarthritis,and is helpful to the diagnosis and treatment of KOA and patient's self-care.2.Gender,age and BMI are significantly related to the characteristics of knee joint disease.The incidence of knee joint disease is higher in females than in males.The older the age,the higher the BMI and the lower the local temperature,the stiffer the knee joint.3.There is a negative correlation between the lateral temperature of patella and WOMAC pain score in patients with cold-dampness obstruction and liver-kidney deficiency.The lower the lateral temperature of patella,the more severe the pain of knee joint.
