Objective:To understand the level of serum uric acid and the prevalence of hyperuricemia in physical examination population in Tianjin.To explore the risk factors of hyperuricemia.To understand the distribution of syndrome elements of hyperuricemia.To provide a certain clinical basis for the prevention and management of hyperuricemia and promoting the standardized diagnosis and treatment of hyperuricemia in traditional Chinese medicine.Methods:(1)A cross-sectional survey was conducted in the physical examination center of the first affiliated Hospital of Tianjin University of traditional Chinese Medicine from June2018 to July 2019,and demographic characteristics and biological indicators were collected.including: physical examiner's name,sex,age,height,weight,body mass index.Vital signs,including blood pressure,heart rate.Life habits,including: smoking history,drinking history.Laboratory indicators,including: liver function index(glutamic pyruvic transaminase,glutamic oxaloacetic transaminase),renal function index(blood urea nitrogen,creatinine,serum uric acid),other biochemical items(fasting blood glucose,triglyceride,total cholesterol,low density lipoprotein cholesterol,high density lipoprotein cholesterol).This paper mainly makes statistics and analysis on the distribution of serum uric acid level in physical examination population and the related factors affecting the distribution of serum uric acid level.In this study,the subjects were grouped and compared according to sex and age.The ages are divided into 13?25,26?35,36?45,46?55,56?65,66?75 and 76?93 years old.The distribution of serum uric acid level in different ages and sexes was compared,and the relationship between physical examination,different living habits,laboratory indexes and serum uric acid level was analyzed.To understand the prevalence and related risk factors of hyperuricemia.(2)Based on the summary of the literature and the dialectical principle of syndrome element dialectics,the questionnaire "TCM symptom questionnaire of hyperuricemia" was made.From March to December 2019,patients with hyperuricemia were diagnosed in the physical examination Center and Rheumatic Immunology Department of the first affiliatedHospital of Tianjin University of traditional Chinese Medicine.The reliability and validity of the questionnaire were tested,the symptoms of hyperuricemia were counted,and the syndrome factors of hyperuricemia were analyzed.Results:(1)(1)There are differences in serum uric acid levels among different age stages,and there are differences in serum uric acid levels between males and females(P<0.05).The overall trend of serum uric acid level in male population decreased with the increase of age,the overall trend of serum uric acid level in female population decreased with age before 45 years old,and increased with age after 45 years old.(2)The level of serum uric acid in smokers and drinkers was higher than that in non-smokers and drinkers(P<0.05).The levels of serum uric acid in hypertension,overweight and obesity,hyperglycemia,high triglyceride,high cholesterol,high and low density lipoprotein cholesterol,low high density lipoprotein cholesterol,elevated glutamic pyruvic transaminase,elevated glutamic oxaloacetic transaminase,elevated blood urea nitrogen and elevated serum creatinine were higher than those in normal people(P<0.001).Body mass index,systolic blood pressure,heart rate,glutamic pyruvic transaminase,glutamic oxaloacetic transaminase,blood urea nitrogen,creatinine,triglyceride,high density lipoprotein cholesterol and low density lipoprotein cholesterol all affect the level of serum uric acid.creatinine,body mass index and low density lipoprotein cholesterol have great influence on the level of serum uric acid.(3)The total prevalence of hyperuricemia was 20.68%.The highest incidence of hyperuricemia occurred before the age of 25 in the overall population.The prevalence ratio of male to female was about 2.8.The prevalence rate of male is different in different age groups,and the prevalence rate is high in the age of 25 and below.The prevalence of hyperuricemia is high in women aged between 66 and 75 years old.(4)Statistical analysis showed that overweight and obesity,hypertriglyceridemia,high and low density lipoprotein cholesterol,elevated glutamic pyruvic transaminase,elevated glutamic oxaloacetic transaminase and elevated serum creatinine were independent risk factors for hyperuricemia.(2)(1)Through the review and analysis of the literature,the questionnaire "TCM symptom questionnaire of hyperuricemia" was made to verify the questionnaire.The structure of thequestionnaire is appropriate,the content is reliable and comprehensive,and it has high reliability and validity.(2)According to the preliminary analysis of the symptoms of hyperuricemia,the top 10 symptoms are: fatigue,fatigue,low back pain,physical fatigue,dry eyes,forgetfulness,spontaneous sweating,blurred vision,sticky mouth and drowsiness.(3)Based on the statistical principle of syndrome element differentiation,the symptoms were induced and transformed into syndrome elements as follows: kidney,liver,spleen,heart,qi deficiency,yin deficiency,dampness,yang deficiency,qi stagnation,blood deficiency,phlegm,heat and blood stasis.It was found that the syndrome factors of hyperuricemia population were analyzed by cluster analysis.Spleen is closely related to dampness,qi deficiency and yang deficiency.Liver,kidney,yin deficiency,blood deficiency or heart are closely related.There is a close relationship between phlegm depression and qi stagnation,and there can also be blood stasis and heat phase.It is often shown as a mixture of deficiency and reality.Conclusion:(1)The level of serum uric acid was correlated with sex,body weight,living habits(smoking and drinking),liver function(glutamic pyruvic transaminase,glutamic oxaloacetic transaminase),renal function(serum creatinine,blood urea),lipid metabolism(triglyceride,high density lipoprotein cholesterol,low density lipoprotein cholesterol).The overall prevalence rate of hyperuricemia in males was higher than that in females,overweight and obesity,hypertriglyceridemia,high and low density lipoprotein cholesterol,elevated glutamic pyruvic transaminase and glutamic oxaloacetic transaminase.Elevated serum creatinine is an independent risk factor for hyperuricemia.(2)The questionnaire "questionnaire on TCM symptoms of hyperuricemia" is suitable in structure,reliable and comprehensive in content,and has high reliability and validity.Based on the preliminary investigation of the questionnaire,it is concluded that the syndrome elements of hyperuricemia include kidney,liver,spleen,heart,qi deficiency,yin deficiency,dampness,yang deficiency,qi stagnation,blood deficiency,phlegm,heat and blood stasis.The disease is a mixture of deficiency and excess,which is characterized by kidney,liver,spleen and heart deficiency,marked as phlegm,heat,dampness and blood stasis.Hyperuricemia is a disease of deficiency and excess.
