Objective: Based on EEG and EMG,the clinical effect of acupuncture on upper extremity dysfunction after stroke was studied,and the objective evaluation method of acupuncture effect was discussed.Methods:From December 2018 to December 2019,40 patients with upper extremity dysfunction after stroke who met the acceptance criteria were enrolled in the rehabilitation medicine department of Tianjin People's hospital.The patients were divided into experimental group and control group by random number table.20 cases in each group.There was no statistical difference between the two groups in the comparison of general data such as age,gender,course of disease,side of disease,etc.Treatment: the experimental group was treated with acupuncture on the basis of conventional rehabilitation,while the control group was treated with conventional rehabilitation only,5 days a week,2 days rest,and 4 weeks treatment cycle.Methods: before and after the treatment,Fugl-Meyer was used to assess the upper extremity(FMA-UE),wolf's upper extremity motor function(WMFT),modified barthle index(MBI)and Brunnstrom's motor function.Before and after the treatment,resting EEG was collected to analyze and compare the average power values of ? wave,? wave,? wave and ? wave.Before and after the treatment,s EMG was collected The i EMG and RMS of biceps,triceps,extensors and flexors were analyzed.Statistical analysis: spss25.0 statistical software was used to analyze the data of this study.? 2 test was used for counting data,paired sample t test was used for measuring data and comparison before and after treatment in the group,and independent sample t test was used for comparison between groups.The rank data before and after treatment were compared by Mann – Whitney U rank sum test,and the intra group comparison by Wilcoxon rank sum test.Pearson correlation analysis was used between EEG,EMG and rehabilitation scale.P <0.05 was considered statistically significant.Results: 1 Analysis of EEG index resultsThere was no significant difference in EEG between the two groups before treatment.After treatment,the average power values of C3,C4 leads,O1,O2 leads ? wave of EEG in the two groups were higher than before treatment,and the increasing range in the experimental group was greater than that in the control group,with statistical significance(P < 0.05);after treatment,the average power values of FP1,FP2 leads,C3,C4 leads ? wave of EEG in the two groups were lower than before treatment,and the decreasing range in the experimental group was greater than that in the control group,There was significant difference between the two groups(P < 0.05),but there was no significant difference among the other leads(P > 0.05).2 Analysis of surface electromyography index resultsThere was no significant difference in i EMG and RMS between the two groups(P > 0.05).After treatment,the RMS values of biceps,triceps,extensor and flexor carpi in the affected side of the two groups were significantly increased(P < 0.01),and the improvement of the experimental group was significantly higher than that of the control group(P < 0.05).3 Analysis of upper extremity function scoreThere was no significant difference in FMA-UE,MBI,WMFT and Brunnstrom score between the two groups before treatment.After treatment,FMA-UE,MBI,WMFT and Brunnstrom score of the two groups were significantly improved compared with those before treatment(P < 0.01),and the improvement range of the experimental group was higher than that of the control group,the difference between the two groups was statistically significant(P < 0.05).4 Correlation analysis of EEG index and upper extremity function scorePearson correlation analysis showed that the ? wave of C3,C4 and O1,O2 leads was positively correlated with FMA-UE and WMFT score(P < 0.05),while the ? wave of FP1,FP2 and C3,C4 leads was negatively correlated with FMA-UE and WMFT score(P < 0.05).5 Correlation analysis of EMG index and upper extremity function score.Pearson correlation analysis showed that the i EMG and RMS of biceps brachii,triceps brachii,extensor carpi and flexor carpi were positively correlated with FMA-UE and WMFT score(P < 0.05).Conclusion:Acupuncture combined with routine rehabilitation therapy has a good effect on improving limb dysfunction after stroke;EEG index,s EMG index and upper limb function score have a good correlation,which can be used as an objective assistant for the evaluation of clinical efficacy of acupuncture in the treatment of upper limb dysfunction after stroke.
