The Establishment And Preliminary Application Of Self-management Efficacy Scale In Patients With Interstitial Lung Disease

?Objects?1.To construct the scale of efficacy of self-management of ILD patients during treatment of interstitial lung disease(ILD),and to test the feasibility,reliability and validity of the scale,so as to provide a more simple,feasible and scientific and effective tool for the evaluation of efficacy of ILD patients' self-management.2.To investigate the status quo of the level of self-management of ILD diagnosed patients during treatment,and to study and analyze the factors affecting the self-management efficiency of this population,so as to provide guidance for the nursing work in the future.?Methods?1.The self-management efficacy scale for patients with interstitial lung disease was constructed based on the theory and task of self-management for chronic diseases,so as to determine the compilation procedures and dimensions of the scale.Through extensive literature retrieval,combined with patient interview data and the self-management scale of chronic diseases developed at home and abroad,the dimensions and item pool of this scale were formed.2.Select and determine the medical and nursing experts in the ILD field in China,and conduct two rounds of expert letter consultation to 19 people by Delphi method,and evaluate and modify the importance of the items in the scale.According to theresults of the second round of letter consultation,the entries were initially selected,and the retained entries formed the first draft of the scale.3.The purposive sampling method was used to jiangsu province,nanjing drum tower hospital 221 ILD diagnose patients as the research object in the scene investigation,survey tool for the first draft of the scale,using frequency distribution method,discrete tendency method,factor analysis,correlation coefficient method,crowns Bach coefficient method for entry five kinds of methods to filter,retain the entry form scale final version.4.The questionnaire was composed of the general data of patients and the self-management efficacy scale(final draft).Using the method of objective sampling,a total of 373 ILD patients from three hospitals in jiangsu province were selected for the field investigation.The reliability of the scale was tested by using kronbhar coefficient,partial and half reliability,and retesting reliability.The content validity and structure validity were used to test the validity of the scale.The efficacy of ILD patients was analyzed,and the factors affecting the efficacy of ILD patients were preliminarily explored.?Results?1.Preparation of scale structureBased on the literature search results and patient interviews,it was determined that the scale included four dimensions,namely,disease information management,daily life management,psychosocial management,and disease treatment management,and 65 items were selected as an alternative entry pool.2.Delphi method resultsThe recovery rates of Delphi expert consultation for the two rounds of scale were91% and 95%,respectively.The expert authority coefficient in the first round of consultation was 0.935,the expert coordination coefficient was 0.266,and the coordination coefficient in the second round was 0.377(P < 0.01).The first draft of the scale was formed according to the expert opinions,and 44 items in the entry pool were retained.3.Screening results of items in the first draft of the scaleThe effective recovery rate of the first draft of the scale was higher,reaching88.4%.After filtering 44 entries in five ways,14 entries were deleted.The dimension of the final draft of the scale remains unchanged,with the remaining 30 items.4.Scale evaluation resultsThe questionnaire recovery rate(acceptance rate)was 93.3%,the completion rate(effective rate)was 100%,and the number of people who completed the scale within10 minutes accounted for 44.7%,indicating that the scale was feasible.Cronbach's resonance coefficient of the scale was 0.754,spearman-brown coefficient was 0.879,fractional half-coefficient was 0.737,and retest reliability was 0.897,indicating that the reliability of the scale was good.The scale SCVI/UA=42/44=0.94,greater than0.9,with good content validity.The KMO value was 0.767,greater than 0.6,indicating that the data had validity.?Conclusion?1.The ILD patients' self-management efficacy scale was constructed in this study,including 4 dimensions and 30 items of disease information management,daily life management,psychosocial management,and disease treatment management.2.This scale has good feasibility,reliability and validity,and can be used as a tool to measure the level of self-management of ILD patients in the future.3.The self-management efficacy of most ILD patients was at a medium to low level after diagnosis,and the main factors affecting the self-management level of this population included gender,age,education,monthly family income,type of accompanying care,and the number of people with respiratory diseases in the family.
