ObjectivePolycystic ovary syndrome(PCOS)was a common heterogeneous endocrine systematic disorder in the female reproductive stage.The main manifestations were hypoovulation/anovulation,hyperandrogenemia,polycystic ovary morphology and so on.It is one of the most frequent cause of ovulation diseases.Women diagnosed as PCOS usually suffer from metabolic dysfunction,which is characterized by high level of insulin resistance,abdominal obesity,high blood pressure and abnormal hyperlipidemia as well as some increased complications during pregnancy and perinatal period.In recent years,assisted reproductive technology(ART)has fundamentally improved the treatment of infertility.Multiple pregnancy is one of the main complications of ART treatment and one of the main causes of adverse pregnancy outcomes.This study aims to analyze the pregnancy outcomes of twins obtained after frozen embryo transfer in PCOS infertile women,and to explore the influence of overweight on the pregnancy outcome of twins obtained after frozen embryo transfer in PCOS patients.MethodsA total of 1449 patients with twin pregnancy obtained after frozen embryo transfer were selected to carry out the retrospective study and analysis because of the factors of polycystic ovary syndrome(PCOS)and tubal infertility in our center of Reproductive Medicine.The PCOS group consisted of 421 infertile women with PCOS and the control group consisted of 1028 infertile women with Fallopian tube related factors at the same time,patients in the two groups were divided into non-overweight(BMI<24kg/m~2)and overweight(BMI?24 kg/m~2)groups according to their body mass index(BMI),then recorded respectively:non-overweight PCOS group(265 cases),overweight PCOS group(156 cases),non-overweight control group(788 cases),overweight control group(240 cases).Comparing the baseline characteristics and pregnancy outcomes of the non-overweight PCOS group and the non-overweight control group,the overweight PCOS group and the overweight control group,the overweight PCOS group and the non-overweight PCOS group,the research indexes include the rate of abortion?early abortion?late abortion?premature delivery?gestational diabetes mellitus(GDM)?hypertensive disorders of pregnancy(HDP)?cesarean section,the gestational week,the average birth weight of the newborn,the rate of the newbornand deformity and so on.Results1.Compared with the non-overweight control group,the abortion rate of the non-overweight PCOS group(16.23%vs9.77%,P<0.05)and the late abortion rate(9.81%vs5.08%,P<0.05)were significantly higher,the gestational week of delivery was earlier(35.71±2.75 weeks vs 36.43±2.07 weeks,P<0.05).The two groups were compared in terms of the rate of early abortion,the rate of preterm delivery,the incidence of HDP?GDM,the average birth weight of the newborn,the rate of neonatal malformation,and the rate of cesarean section,the difference was not statistically significant.2.Compared with the overweight control group,the abortion rate(19.87%vs 11.25%,P<0.05)and late abortion rate(12.18%vs 6.25%,P<0.05)of the overweight PCOS group were significantly higher,and the pregnancy week of delivery significantly earlier(35.06±2.77 weeks vs 36.19±1.87 weeks,P<0.05),the average birth weight of newborns was significantly reduced[2350.00(602.50)g vs 2380.00(677.50)g,P<0.05].There was no significant difference in early abortion rate,preterm birth rate,HDP rate,GDM rate,cesarean section rate,neonatal malformation rate between the two groups.3.Compared with the non-overweight PCOS group,the gestational week of delivery was earlier in the overweight PCOS group(35.06±2.77 weeks vs 35.71±2.75 weeks,P<0.05),and the average birth weight of newborns decreased[2350.0(602.50)g vs2390.00(550.00)g,P<0.05],the difference is statistically significant.On the other hand,there was no significant difference in the study of abortion rate,early abortion rate,late abortion rate,preterm birth rate,HDP rate,GDM rate,cesarean section rate,neonatal malformation rate.ConclusionThe rate of abortion and late abortion may increase after frozen embryo transfer in infertile women with polycystic ovary syndrome(PCOS),at the same time,the birth weight of newborns in overweight women with PCOS may be decreased.
