Clinical Outcome Analysis Of Vitro Fertilization-Embryo Transfer, Ovulation Induction And Natural Conception In Twin Pregnancies

Objective:To investigate the effects of vitro fertilization-embryo transfer,ovulation induction and natural pregnancy on perinatal complications,gestational weeks,mode of delivery and neonatal outcomes in twin pregnancies.Methods:A total of 352 cases of twin-term pregnant women who were examined and delivered in our hospital regularly from December 2012 to February 2019 were collected.According to their different methods of conception,it can be divided into in vitro fertilization-embryo transfer(IVF-ET)group,ovulation promotion(OI)group and natural conception(NC)group,and their clinical data were collected by professionals,including the general situation of pregnant women,gestational weeks and delivery methods,perinatal complications,and neonatal outcomes.SPSS21.0 system was used for statistical analysis.Results:1.There was no significant difference in the age,pregnancy,parity,pregnancy weight gain and BMI of pregnant women with different conceptions(P>0.05).2.The incidence of hypertensive disorder complicating pregnancy and intrahepatic cholestasis of pregnant women in IVF-ET group was higher than that in OI group and NC group(P<0.05).There was no difference in OI group and NC group.Academic significance(P>0.05).The incidence of hypothyroidism in pregnant women was higher than that in the OI group and the NC group(P<0.05).The incidence of gestational diabetes in the OI group was higher than that in the other two groups(P<0.05).There was nosignificant difference between the IVF-ET group and the NC group(P>0.05).There was no significant difference in the incidence of complications such as anemia,placenta previa,arrhythmia,gestational thrombocytopenia,placental abruption and postpartum hemorrhage during pregnancy(P>0.05).3.The average gestational age(33.0±2.4 weeks)in the IVF-ET group was lower than that in the OI group(35.9±2.3 weeks)and the NC group(36.2±2.3 weeks),the difference was statistically significant(P<0.05).There was no significant difference between the OI group and the NC group(P>0.05).There was no significant difference in the mode of delivery between pregnant women with different conceptions(P>0.05).4.There were no significant differences in the different methods of conception in neonatal jaundice,low birth weight infants,neonatal asphyxia,alpha fetus weight,birth weight,and fetal malformation(P>0.05).5.Comparisons of perinatal complications and neonatal outcomes of twin pregnancies aged less than 35 years and older than 35 years among groups:The age of pregnant women is less than 35 years: The mean gestational age of the IVF-ET group was smaller than that of the OI group and the NC group,the difference was statistically significant(P<0.05).There was no significant difference between the OI group and the NC group.The incidence of hypertensive disorder complicating pregnancy and intrahepatic cholestasis of pregnant women in IVF-ET group was higher than that in the other two groups(P<0.05).There was no significant difference between OI group and NC(P>0.05).The incidence of gestational diabetes in the OI group was higher than that in the other two groups(P<0.05).There was no significant difference between the IVF-ET group and the NC group(P>0.05).There was no significant difference in the incidence of asphyxia between the three groups of neonates in jaundice,low birth weight infants and neonates(P>0.05).The age of pregnant women is older than 35 years:the average gestational age in the IVF-ET group was smaller than that in the OI group and the NC group(P<0.05).There was no significant difference between the OI group and the NC group(P>0.05).The incidence of hypertensive disorder complicating pregnancy in the IVF-ET group was higher than that in the OI group and the NC group(P<0.05).There was no significant difference between IVF-ET group and NC group,and between OI group and NC group(P>0.05).The incidence of hypothyroidism in pregnant women in IVF-ET group and OI group was significantly higher than that in NC group(P<0.05).There was no significant difference between IVF-ET group and OI group(P>0.05).In the IVF-ET group,the intrahepatic cholestasis of pregnant women was higher than that of the other two groups(P<0.05).There was no significant difference between the OI group and the NC group(P>0.05).There was no significant difference in the incidence of asphyxia in children with jaundice,low birth weight,and neonatal asphyxia(P>0.05).6.Group comparison:The incidence of intrahepatic cholestasis and placenta previa in IVF-ET group was higher than that in the group of pregnant women aged less than 35years(P < 0.05).The incidence of gestational hypertension and gestational diabetes mellitus in OI pregnant women aged over 35 years was higher than that in pregnant women aged less than 35 years(P < 0.05).The incidence of placenta previa and neonatal asphyxia in OI pregnant women aged over 35 years was higher than that in pregnant women aged less than 35 years(P < 0.05).The incidence of neonatal asphyxia in NC pregnant women aged more than 35 years was significantly different from that in pregnant women aged less than 35 years(P < 0.05).There was no significant difference in the incidence of perinatal complications between NC pregnant women and pregnant women aged less than 35 years(P > 0.05).Conclusion:1 In vitro fertilization-embryo transfer may increase the risk of hypertensive disorder complicating pregnancy,intrahepatic cholestasis of pregnancy and hypothyroidism during pregnancy.2 In vitro fertilization-embryo transfer and ovulation induction do not increase the risk of adverse outcomes in neonates.3 Elderly pregnant women were associated with perinatal complications and neonatal adverse outcomes.4 The management and health care during the perinatal period should be strengthened for twin pregnancy,especially for IVF-ET,OI and older pregnant women.
