Objective To investigate the clinical features and imaging features of musculoskeletal ultrasound in patients with chronic elbow pain.By analyzing its two-dimensional ultrasound,color Doppler and other ultrasound features,combined with ultrasound-guided interventional therapy,in order to explore the value of musculoskeletal ultrasound in the differential diagnosis and treatment of chronic elbow pain disease.Methods From January 1,2015 to March 31,2018,699 patients with chronic elbow pain were selected,a total of 730 elbow joints were examined by musculoskeletal ultrasound.By recording the characteristics of joint,muscle and tendon tissue sonogram,color Doppler blood flow distribution,combined with the patient’s age,sex,history and other clinical data,epidemiological characteristics and imaging analysis.Among them,75 patients underwent real-time ultrasound guided interventional therapy to analyze its therapeutic effect.Results Chronic elbow pain in 699 patients was divided into youth,middle age and old age groups,accounting for 36.0%,50.4% and13.6%,respectively.The incidence of external epicondylitis of humerus(tennis elbow)was the highest in ultrasound diagnosis,which incidence rate is 47.2%,incidence of elbow arthritis is 22.9%,incidence of medial humeral epicondylitis is 3.3%,Incidence of cysts is 9.8%,Incidence of Cubital tunnel syndrome is 4.7%,incidence of lymphadenopathy on the pulley is 5.8%,incidence of collateral ligament injury is 6.3%.Compared with the youth group,the incidence of extensor tendon lesions and elbow arthritis was higher in the middle and old groups(P<0.05).The incidence of elbow arthritis varies between men and women in the sexgroup(P<0.05).On the left and right sides of the elbow,there were significant differences in medial epicondylitis of the humerus,cubitus syndrome and lymphadenopathy of the trochlear(P<0.05).Musculoskeletal ultrasound can be used to treat diseases under the guidance of real-time ultrasound.In 699 patients,75 patients underwent real-time ultrasound guided interventional therapy.Of these,45 LE patients were treated with interventional ultrasound.the total effective rate was 88.9%,and the curative effect was significantly better than that of the control group significant(P<0.05).A total of 15 cases of olecranon bursitis were aspirated and injected under interventional ultrasound,the total effective rate was 86.7%,and the curative effect was significantly better than that of the control group significant(P<0.05).Interventional ultrasound nerve block therapy was performed in 15 patients with cubitus syndrome,the total effective rate was 93.3%,the pain relief rate was 88.9%,and the curative effect was significantly better than that of the control group significant(P<0.05).Conclusion Musculoskeletal ultrasound has good clinical value in imaging diagnosis of chronic elbow pain.The diagnosis of elbow pain is helpful by clear and simple ultrasonic examination.For patients with chronic elbow pain,two-dimensional ultrasound images and color Doppler blood flow in the lesion have characteristic manifestations,which provide objective reference for disease diagnosis,clinical choice of treatment,evaluation after treatment,and withdrawal time.At the same time,ultrasound-guided pain interventional therapy can be carried out in real time and accurately under ultrasound,reducing the complications caused by blind puncture or inaccurate positioning.Therefore,musculoskeletal ultrasound in the diagnosis of chronic elbow pain disease,not only can achieve the differential diagnosis of the disease,color Doppler ultrasound and energy Doppler technology in the treatment of follow-up and evaluation of curative effect has a very good clinical guidance. |
