Objective: To explore the comparative effects of the concept of enhanced recovery after surgery(ERAS)and conventional treatment methods in the perioperative period of unilateral primary total hip arthroplasty(THA)in middle-aged and elderly patients.Methods: A total of 135 patients with artificial THA who met the criteria for inclusion in Department of Orthopaedics of the Affiliated Hospital of Guilin Medical College from July 2017 to October 2019 were selected.There were 61 patients in the experimental group adopted the perioperative treatment method based on the concept of ERAS.There were 74 patients in the control group treated with conventional perioperative treatment.The clinical data of the two groups of patients were analyzed retrospectively,which include and the pain evalutated by visual analogue scale(VAS)after THA,perioperative complications,number of postoperative infections,number of postoperative blood transfusions,patients' satisfaction,postoperative Oxford hip score(OHS)at 2 weeks and 2 months(The score is between 0-48,the normal hip joint score is 48 points)and length of stay(LOS).Results: Pain VAS score [(3.64 ± 1.07)] in the experimental group at 1 day after operation was better than that in the control group [(4.29 ± 1.20)];the number of nausea and vomiting [16/74] in the experimental group was less than that in the control group [5/61] during the perioperative period;24 hours postoperative THA Blood oozing drainage at the surgical site in the experimental group [(183.28 ± 30.56)ml] is better than that in the control group [(207.35 ± 32.02)ml];the number of postoperative blood transfusions in the experimental group [6/61] was less than that in the control group [18/74];by compared with the patient satisfaction rate,the experimental group satisfaction [93%] is higher than that in the control group [77%];OHS 2 weeks after operation in the experimental group [(34.25±2.97)] is better than that in the control group [(29.97±2.94)];the average LOS in the experimental group [(9.98 ± 3.39)days] was lower than that in the control group [(11.48 ± 4.27)days];However,there was no statistical significant difference between the two groups of THA patients in terms of VAS at 3 day after operation,incision infection,urinary system infection,lung infection,venous thromboembolism(VTE),and 2 month postoperative OHS(P>0.05).Conclusions: The application of the concept of ERAS in the perioperative period of unilateral primary THA in middle-aged and elderly patients is superior to conventional treatment.Worthy of clinical application.
