Clinical Observation Of Juanyin Xiefei Recipe On Airway Mucus Hypersecretion Of AECOPD With Phlegm-heat Obstructing Lung Syndrome And Its Effect On Respiratory Mechanics

Objective:To observe the clinical effect of Juanyin Xiefei Decoction combined with western medicine on airway mucus hypersecretion and respiratory mechanics in patients with acute exacerbation of chronic obstructive pulmonary disease?COPD?of phlegm-heat obstruction of lung syndromeMehods:76 patients with acute exacerbation of chronic obstructive pulmonary disease who met the inclusion criteria were divided into treatment group?38 cases?and control group?38 cases?.The control group was treated with Western medicine,including anti-infection,cough-relieving and phlegm-resolving,bronchodilator,etc.The Treatment group was treated with Juanyin Xiefei Decoction and Western medicine.After one week of treatment,the clinical efficacy,TCM syndrome integral,cough and Sputum Assessment questionnaire,CAT scale,inflammatory factors and the effect of respiratory mechanics were observed.Results:1.After treatment,the total effective rate was 94.3%in the treatment group and78.0%in the control group.2.Compared with the control group,the total effective rate in the treatment group was better than that in the control group?P<0.01?.The total scores of TCM syndromes in the two groups were improved compared with those before treatment?P<0.01?,and the group with combination of traditional Chinese and western medicine was better than the group with western medicine?P<0.01?.At the same time,the single scores of TCM syndromes in the two groups including cough,expectoration,wheezing,chest tightness,fever,constipation and thirst menstruation were improved after comparison?P<0.01?.After comparison between the two groups,the treatment group improved cough,expectoration,wheezing,and constipation Knot and thirst were better than those of the control group?P<0.05?.There was no difference in fever and chest tightness between the two groups.3.The scores of CASA-Q questionnaires in both groups were higher than those before treatment?P<0.01?,However,there was no significant difference between the two groups in cough influence and Sputum influence after treatment?P?0.05?.The CAT scores of group C and group D in the two groups were lower than those before treatment?P<0.01?,and the reduction degree of the treatment group was better than that of the control group?P<0.05?.4.In two groups of inflammatory factors,serum interleukin 6?IL-6?and tumor necrosis factor-??TNF-??level by comparison within groups and between groups were no statistically significant difference?P>0.05??5.In terms of respiratory mechanics,the level of IC?L?in the treatment group had statistical difference and better effect than that in the control group?P<0.05?.The levels of FEV1/FVC%,FEV1?L?,FVC?L?and FEV3?L?were all improved after treatment,but only the improvement of FEV3?L?was better than that of control group?P<0.01?.The levels of MVV?L/min?and VC?L?were significant differences in the treatment group?P<0.05?,but there was no difference between the two groups?P>0.05?.The levels of MEF50?L/S?,MEF75?L/S?and MMEF?L/S?,only the improvement level of MEF75?L/S?was better than that of the control group?P<0.05?.There were no significant differences in R5 cm H₂O?L/S?,R20 cm H₂O?L/S?,R5-R20cm H₂O?L/S?,X5 cm H₂O?L/S?,total resistance Zrs cm H₂O?L/S?and resonance frequency between the two groups?P>0.05?.6.In terms of safety,there were no abnormal changes in blood routine,liver and kidney function before and after treatment in the two groups.ConclusionJuanyin Xiefei Decoction combined with western medicine can improve the syndrome of phlegm-heat obstructing lung in acute exacerbation of chronic obstructive pulmonary disease,alleviate the clinical symptoms of airway mucus hypersecretion,improve the quality of life,and influence the parameters of respiratory mechanics,especially in improving the levels of FEV3?L?,IC?L?and MEF75?L/S?.
