Objective:From the point of view of animal model,this paper discusses the mechanism of Shi's Plaster on rabbit knee osteoarthritis synovitis.This experimental provides reliable experimental data for the clinical application and development of this plaster.Methods:Forty male New Zealand rabbits were randomly divided into four groups: blank group,model group,Shi's diaoshao Plaster group,Compound Zijing Plaster group,with 10 rabbits in each group.An experimental rabbit model of osteoarthritis synovitis was established by 4% papain and 0.03mol/L l-cysteine mixture(2:1)injection.The modeling time was 14 days.After successful modeling,no treatment was given to the model group and the blank group.Plasters are used in the Shi's Plaster group and the Compound Zijing Plaster group with eight-figure knee joint bandages respectively.The intervention treatment lasted for 14 days.In the whole experiment process,joint surface condition,walking state and skin temperature were observed and measured,and the knee joints of experimental rabbits after modeling and intervention were examined by MR.After the rabbits were killed,the serum,synovial fluid and synovium of knee joint were collected.The pathological changes of synovial tissue were observed by HE staining,the concentrations of TNF-?,IL-1 ? and PGE2 in synovial fluid were measured,the concentration of nesfatin-1 in serum and the expression of nesfatin-1 in synovial tissue were measured.Immunofluorescence analysis of AQP1 and 4 paraffin sections of synovial tissue.Results:1.The rabbits injected with drugs showed swelling of the right knee joint,red complexion and elevated skin temperature,and at the same time,the knee joints were involved,such as lameness,protective leg contraction and so on.MR in the model group showed increased fluid accumulation in the knee joint and wear and tear of cartilage,hyperemia and edema of synovium,more turbidity of synovial fluid,partly gelatinous,yellow or red,and obvious proliferation of synovial cells in the pathological section of synovium.The pathological manifestations of synovitis covered more than 5 cell layers,the number of stromal cells increased and a large number of inflammatory cells infiltrated,compared with the blank group,the levels of TNF-?,IL-1 ? and PGE2 in synovial fluid were significantly increased,and the content of nesfatin-1 in serum was significantly decreased.The expression of nesfatin-1 decreased and the expression of AQP1 and 4 increased in synovium.2.Compared with the model group,the redness and swelling of the knee joint was obviously relieved in the two intervention groups.MR showed that the fluid of the knee joint was absorbed after intervention,and there was obvious wear on the cartilage surface in the model group,but there was no obvious wear in the other groups.The swelling of the experimental rabbits in the Shi's diaoshao Plaster group was basically eliminated and they walked normally.MR showed that the accumulated fluid of knee joint was basically normal.The swelling of knee joint in Zijing Plaster group was basically eliminated,and some rabbits had the phenomenon of decreased desire to walk.MR in some rabbits showed that there was still slightly unabsorbed pathological fluid.3.The contents of TNF-??IL-1?and PGE2 were significantly decreased after intervention.There was no significant difference in the concentration of the three inflammatory factors between the two groups,no significant difference in the IL-1? concentration between the two groups,and no significant difference in the concentration of PGE2 between the two groups.4.Compared with the model group,nesfatin-1 concentration increased in serum and synovial membrane after intervention.The serum nesfatin-1 concentration in Compound Zijing Plaster group was higher than that in Shi's diaoshao Plaster group,but the difference in the synovial content was smaller.5.Compared with the blank group,the expression of AQP1 and 4 in the synovium of the model group was significantly increased;compared with the model group,the expression of AQP1 and 4 in the Shi's diaoshao Plaster group and Zijing Plaster group was significantly decreased,the difference was statistically significant.Conclusion:1.The model of knee osteoarthritis synovitis was successfully established.2.Shi's diaoshao Plaster can eliminate joint cavity effusion,relieve joint inflammation of joint swelling and heat,cure the knee osteoarthritis synovitis,and make the rabbits walking normal.3.The mechanism of Shi's diaoshao Plaster: By reducing the concentration of TNF-??IL-1?and PGE2 in synovial fluid,reducing the concentration of AQP1 and 4,increasing the concentration of Nesfatin-1 in serum and increasing the expression of Nesfatin-1 in synovium.In order to reduce the infiltration of inflammatory cells and restore the normal proliferation of synovial cells.4.Compared with compound bauhinia anti-injury ointment,hanging wound ointment can reduce the skin temperature of knee joint more quickly,and the effect of detumescence is stronger,but there is no significant difference in the rest.
