Objective:To observe the clinical effect of xiyanping injection on mycoplasma pneumoniae pneumonia in children and explore its mechanism.Methods:1.Using a retrospective case-control study to analyze the clinical efficacy.30 children with mycoplasma pneumoniae pneumonia treated with xiyanping injection and azithromycin were selected as the case group,and 30 children hospitalized with mycoplasma pneumoniae pneumonia treated with azithromycin were selected as the control group according to the ratio of 1:1.2.Through the network pharmacology research method,the TCMSP,BATMAN and OMIM databases were mined to obtain the targets of xiyanping injection and mycoplasma pneumoniae.Cytoscapev3.5.1 software and its Ceti Scape plug-ins were used to construct protein interaction(PPI)network,and the key network was selected,the KEGG pathway enrichment analysis of the key targets was carried out by DAVID database,and the potential regulatory pathway of xiyanping injection in the treatment of mycoplasma pneumoniae was screened out to explore its mechanism of action.3.Conducting animal studies to construct a mouse model of mycoplasma pneumoniae pneumonia,mice were randomly divided into 5 groups(solvent control group,MPP modeling group,xiyanping injection-L group,xiyanping injection-H group,azithromycin group).The behavioral indicators of fever and cough were observed after drug intervention,the values of CD4~+T,CD8~+T and CD4~+/CD8~+T of alveolar lavage fluid and serum T cell subsets,as well as the serum IL-17 concentration,were detected in each group,so as to verify the effectiveness of xiyiping injection in the treatment of mycoplasma pneumoniae pneumonia and explore its mechanism of action.Results:1.Clinical study results showed that the curative effect of the disease in the case group was better than that in the control group,the duration of hospitalization of the fever,cough and lung rales in the case group was shorter than that in the control group,and the incidence of adverse reactions in the case group was lower than that in the control group,with statistical significance(P<0.05).2.Network pharmacology studies showed that xiyanping injection had 287 targets and mycoplasma pneumoniae pneumonia had 196 targets,which could be constructed into PPI network graph composed of 427 points and 2847 edges.The key network composed of 107points and 1501 edges is further selected.Finally,the KEGG pathway enrichment analysis was carried out to screen out 10 immune system-related pathways,among which the IL-17signaling pathway contained the most key targets。3.Animal experiments showed that xiyanping injection can effectively relieve the elevated body temperature,increased cough times and lung tissue lesions in mice with mycoplasma pneumoniae pneumonia.The imbalance of CD4~+T and CD8~+T cells in T cells caused by MP infection was significantly improved and down-regulate the expression of IL-17.Conclusion:1.Preliminary clinical results showed that xiyanping injection combined with azithromycin in the treatment of mycoplasma pneumoniae pneumonia in children could significantly improve the curative effect.2.Xiyanping injection can effectively relieve the temperature rise of mycoplasma pneumoniae pneumonia mice,inhibit the increase of cough times and improve the lung tissue lesions,and its mechanism of action may be related to down-regulating the expression of IL-17,restoring the proportion of CD4~+T and CD8~+T cells,and regulating the immune dysfunction.
