ObjectiveTo understand the composition ratio of mycoplasma pneumoniae pneumonia(MPP)and refractory mycoplasma pneumoniae pneumonia(RMPP),age and month of prevalence in Xi’an Children’s Hospital.to explore the timing of the administration of glucocorticoid therapy by RMPP,trying to provide a treatment plan for RMPP in clinical practice.MethodsA retrospective study was used to collect clinical data of community-acquired pneumonia(CAP)hospitalized children in the Respiratory Department of Xi’an Children’s Hospital from January 2017 to December 2019,selecting children who meet the diagnostic criteria of MPP and RMPP from them and understanding the composition ratio of MPP and RMPP,age and month of occurrence.The children with RMPP included in the study were divided into the early treatment group and the late treatment group,according to the day or night of hormone therapy.The curative effects of the two groups of children before and after the hormone treatment were compared to explore the timing of hormone treatment.Results1.There were 1,414 cases of MPP in 5,814 cases of CAP,accounting for 24.32% of CAP.The average age of children with MPP is(5.41±2.79)years old,the smallest is 0.08 years old,the largest is 16 years old;<1 year old: 47 cases;1 year old ≤ age <3 years old:257 cases;3 years old ≤ age <7 years old:687 cases;≥7 years old: 423(of which 85cases> 10 years old);3-10 years old children are more common,a total of 1025 cases.MPP usually occurs from July to December(70.30%).2.There were 177 cases of RMPP in 1,414 cases of MPP,accounting for 12.52% of MPP.The average age of children with RMPP is(7.03±2.33)years old,the youngest is 2.17 years old,the oldest is 16 years old;<1 year old: 0 cases;1 year old ≤ age <3 years old: 4cases;3 years old ≤ age <7 years old: 86 cases;≥ 7 years old: 87cases(including 17 cases>10 years old).RMPP usually occurs from October to December(50.28%).3.The curative effects of the two groups of children in RMPP who before and after giving hormone treatment showed that the early treatment group had fever time 3.00(3.00,3.00)days after admission,cough improvement time 9.00(7.00,10.00)days,lung rales disappeared time 9.00(8.00,11.00)days,hospitalization time 11.00(9.75,13.25)days were less than 5.00(4.00,5.00)days,10.00(9.00,12.00)days,11.00(10.00,13.00)days,12.00(11.00,14.00)days in the late treatment group after admission in the two groups,there was a statistically significant difference in the clinical manifestations of the two groups of children after admission(P<0.05);The early treatment group combined with 11 cases of abnormal liver function,11 cases of hypoproteinemia,6 cases of myocardial damage,28 cases of pleural effusion,are less than 24 cases,24 cases,16 cases,and 46 cases of the late treatment group,there were statistical differences in the appearance of extrapulmonary complications in children(P<0.05);The average hospitalization fee of the early treatment group(12441.55±3827.07)yuan was lower than the average hospitalization fee of the late treatment group(13781.51±3604.90)yuan,there there was a statistical difference in the average hospitalization fee between the two groups(P<0.05).4.The comparison of inflammatory indexes in the before and after the hormone administration of RMPP showed that CRP and PCT of children in the before and after hormone administration: 67.80(23.89,103.66)mg/L and 0.72(0.10,0.64)mg/L,were greater than 11.30(5.00,14.31)mg/L and 0.11(0.05,0.10)ng/L,after hormone administration,There were statistical differences in CRP and PCT before and after the use of hormones in children(P<0.05).Conclusion1.MPP who from January 2017 to December 2019 in the First Respiratory Department of Xi’an Children’s Hospital accounted for 24.32% of CAP;it is more common in children aged 3-10 years old,and it usually occurs from July to December.2.RMPP accounts for 12.52% of MPP;it is more common in children aged 3-10 years old,and it usually occurs from October to December.3.RMPP administration of methylprednisolone sodium succinate treatment can alleviate the inflammatory response;administration of methylprednisolone sodium succinate treatment RMPP within 4 days of admission can shorten the fever heat course,cough time,disappearance time of lung rales,hospital stay,and extrapulmonary complications.Further more,it can also reduce hospitalization costs.
