Analysis The Rule Of Medication Used By Chief Physician LiLanqing In Treating Lower Limb Erysipelas With TCM Based On Data Mining

Objective:To explore the medication rule of Director Li Lanqing in the treatment of patients with lower limb erysipelas disease,observe the clinical effect and analysis of prescription drugs to the formula,the principle of drug prescription and treatment ideas.The aim is to pass on the essence and keep the innovation,so as to provide the prescription and theoretical basis for improving the clinical effect of TCM on lower extremity erysipelas.Methods: Experiment 1:A tolal of 207 effective prescriptions of TCM were collected from from 74 patients who were diagnosed as lower limb disease by Li Lan Qing,chief physician of the department of canker and department of surgery,affiliated hospital of tianjin academy of traditional Chinese medicine from February 2015 to February 2019.After the data is standardized,the data were recorded in Excel to form a database,and the preliminary statistics on the frequency of medication were conducted.Descriptive statistics were made on the four properties,five flavor,meridian tropism,and efficacy of the drugs used are counted,and the results were obtained to form a chart.The high-frequency drugs in prescriptions were imported into SPSS 25.0 software for cluster analysis,and new drug combinations were obtained.This paper analyzed the application frequency of drugs and their properties,taste,meridien,and efficacy,and the results of the prescription drug pair obtained after cluster analysis.The experience and ideas of the treatment of lower limb disease and the rules of medication were summarized.Applied to the clinic.Experiment 2: from May 2019 to May 2020,18 patients with acute stage of Erysipelas were diagnosed as Lower Limb Erysipelas by Li Lanqing,chief physician of Tianjin University of Traditional Chinese Medicine.Li Lanqing,the chief physician,was treated with the decoction of dispersing Dan and detoxicating orally for 7 days.The Clinical Curative Effect was observed by self-control method,the changes of clinical efficacy,Symptom score and inflammatory index were observed and compared separately before and after treatment.Results: Experiment 1:1 A total of 74 patients were included,with 207 effective TCM prescriptions.The ratio of men to women is 1.6 to 1;The mean age was 60.80±14.53 years.2 Among the effective prescriptions for the treatment of lower extremity erysipelas,a total of 141 drugs were used.Among them,the frequency of drugs with frequency more than 50 times is Honeysuckle,132 times;,Alisma,170 times;Poria Cocos,169 times;Coicis Seed,114 times;Atractylodes Macrocephala,95 times;Prunella Vulgaris,87 times;Herba Oldenlandiae,86 times;Fructus Kochiae,51 times;Dandelion,106 times;Wild Chrysanthemum,62 times;Tu Fuling,98 times;Tangerine Peel,129 times;Red Peony Root,186 times;Fang Feng,131 times;Chinese Starjasmine Stem,66 times;Caulis Spatholobi,52 times;Achyranthes,189 times;Moutan Bark,82 times;Rehmannia,70 times;Scutellaria,113 times;Rice Bean,73 times.3 Analyze all the drugs used.The frequency of use of the four properties from high to low is cold,warm,flatness,cool,and heat.4 According to the classification of five flavors,the frequency of use from high to low for sweet,bitter,spicy,light,sour,salty,astringency.5 The five most frequent meridians are liver meridian,spleen meridian,lung meridian,kidney meridian,and stomach meridian.6 The five drugs with the highest frequency of efficacy are heat-clearing medicine,waterwetting medicine,blood-activating medicine,tonic medicine,and antisuperficial drugs.7 Import the data into SPSS 25.0 software for cluster analysis,and get the more common drug pairs: Group1: Poria Cocos,Alisma;Group2:Chinese Starjasmine Stem,Caulis Spatholobi;Group 3: Prunella Vulgaris,Achyranthes;Group 4:Coicis Seed,Rice Bean;Group 5: Tangerine Peel,Scutellaria;Group6: Fructus Kochiae,Fang Feng Group 7: Herba Oldenlandiae,Dandelion;Coicis Seed,Rice Bean;Group8:Honeysuckle,Tu Fuling,Peonies;Group9: Wild Chrysanthemum,Rehmannia.Experiment 2:1 of the 18 patients included,the mean age of the patients was 65±14.32 years,the maximum age was 88 years,the minimum age was 31 years.2 Clinical Curative Effect Comparison: 18 cases of acute stage of Erysipelas in lower limbs were included,6 cases were cured,0 cases were ineffective,the effective rate was 100%.3 comparison of laboratory indexes: After treatment,WBC,neut% and CRP were significantly lower than before treatment.4 Comparison of Symptom Score: After treatment,the total score of symptom and sign(fever,fatigue,headache,dry and bitter mouth,yellow urine and affected side Groin lymphadenopathy)was significantly lower than that before treatment(p<0.01)After treatment,the skin color,skin temperature,swelling and pain of the patients were improved obviously,the difference was statistically significant(p<0.01),the difference was statistically significant(p<0.01).Conclusion:1 Experiment 1: A total of 207 prescriptions were included in this study,141 Chinese herbs were used,the total frequency of use 3436 times,a total of 16 categories.(1)The frequency of core drugs: 21 drugs with more than 50 times;(2)Drug of four properties: cold drugs use frequency is most,followed by warm drugs and peaceful drugs;(3)Five flavors of drugs: sweet,bitter and spicy drugs are frequently used;(4)Meridian of drugs: liver,spleen,lung,kidney and stomach are in the forefront;(5)Classification of drug efficacy: The frequency of drug use with frequency higher is followed by heat-clearing medicine,water-wetting medicine,bloodactivating medicine,tonic medicine,antisuperficial medicine.There are 9 groups of new drug combinations:2 Experiment 2: Li Lanqing,chief physician,experienced treatment of Erysipelas in the acute stage of lower limbs by taking orally the traditional Chinese Medicine Fangshandanjiedu decoction,which can improve the systemic and local symptoms and signs and inflammatory indexes,with an effective rate of 100%.3 Summary of chief physician Li Lanqing's experience:In the treatment of Erysipelas Diseases,director Li used heat-clearing,dampness-removing,blood-activating,warm and firmdispersing as the main therapeutic methods.Director Li prescribed drugs to distinguish the urgency of the disease,syndrome differentiation and treatment features prominent reconciliation of the zang-fu,reinforcing deficiency and strengthening the essence,differentiation of damp-heat,promoting blood circulation,removing blood stasis and Internal and external treatment.
