OBJECTIVE: To investigate the clinical effect and influencing factors of acupuncture and moxibustion combined therapy in different Parts of cerebral infarctionMETHOD: This study is a retrospective cohort study.Data collection and statistical analysis were carried out on the cases that met the study standards from January 2017 to December 2018 in the acupuncture department of The first affiliated hospital of tianjin university of traditional Chinese medicine SPSS25 software was used.Logistic regression model was established by using exposure factors as covariables to explore the clinical efficacy of acupuncture combined therapy for Patients with anterior and Posterior cerebral infarction.RESULT: ?1 In the data of this study,there were statistically significant differences in the scores of the NIHSS scale and the muscle strength scores of the upper and lower limbs of the Patients with cerebral infarction on admission(OR=1.315?95% CI 1.091-1.587?P=0.004 < 0.05,OR=1.236?95% CI 1.020-1.498?P=0.030).The difference in the scores of NIHSS scale was statistically significant(OR=1.225 ? 95% CI 1.002-1.571 ? P=0.048)in the intervention of acupuncture and moxibustion in different parts of cerebral infarction,and the comprehensive treatment of acupuncture and moxibustion was superior to that of posterior circulation cerebral infarction in the improvement of neurological functional defects in Patients with anterior circulation cerebral infarction;?2 NIHSS scale score of Patients in various course and upper and lower limb muscle strength grade differences statistically significant(P < 0.05),the course of the disease in Patients with in 7 days more or less showed mild nervous function and limb muscle strength barriers,that is,the occurrence of acute Phase of cerebral infarction in the early of nervous function and body is relatively light dysfunction,comprehensive acupuncture therapy intervention time in improving patient NIHSS scale score and limb muscle strength score differences statistically significant(P < 0.05),the comprehensive acupuncture and moxibustion therapy on the improvement of neural function and Physical function in Patients with the best intervention time for the onset of 8 to 14 days,That is to say,acupuncture and moxibustion combined therapy in the late stage of acute cerebral infarction can improve the nerve function and limb function of patients;?3 Age had a statistically significant difference in the scores of acupuncture and moxibustion combined therapy intervention to improve the NIHSS scale.Among patients younger than 70 years old,the younger the age,the better the effect of acupuncture and moxibustion combined therapy intervention to improve the neurological function.Age had statistically significant difference in limb muscle strength score of Patients(P < 0.05),and mild limb muscle strength disorder was more likely to occur between 51 and 60 years old,and acupuncture and moxibustion combined therapy intervention had better clinical efficacy in improving limb muscle strength;?4 There was no statistically significant difference between gender in NIHSS scale score and lower limb muscle strength score of patients with cerebral infarction(OR=1.038 95%CI0.854-1.262 P > 0.05,OR=0.971 95% CI 0.794-1.188 P=0.775),and the difference in upper limb muscle strength score was statistically significant(P < 0.05).Gender had no statistically significant difference in the score of NIHSS scale and muscle strength score of upper and lower limbs improved by acupuncture intervention(OR=1.029,95% CI 0.845-1.252 P=0.775,OR=1.082,95%CI 0.782-1.498 P=0.632,OR=0.903,95%CI 0.678-1.201 P=0.484);?5 Comprehensive therapy of acupuncture time improving Patient NIHSS scale score and upper and lower limb muscle strength score differences statistically significant(P < 0.05),within 60 days,the longer comprehensive acupuncture therapy in improving the curative effect of patients with lower limb function may be better,but the comprehensive acupuncture therapy time within 14 days to improve neural function in patients with poor clinical curative effect and function of upper extremity.CONCIUSION: ?1 Anterior circulation cerebral infarction is more serious than posterior circulation cerebral infarction in the degree of affecting nerve dysfunction and limb dysfunction,and acupuncture and moxibustion combined therapy is better than posterior circulation cerebral infarction in improving the neurological impairment of patients with anterior circulation cerebral infarction;?2 The early stage of the acute stage of cerebral infarction appears relatively mild neurological function and limb dysfunction,in the late stage of the acute stage of cerebral infarction to give acupuncture and moxibustion comprehensive therapy intervention to improve the patient's neurological function and limb function curative effect is better;?3 The Patients aged from 51 to 60 years old are more likely to have mild limb muscle strength disorder,and acupuncture and moxibustion combined therapy intervention has a good clinical effect on the improvement of lower limb muscle strength.In patients younger than 70 years old,the younger the age,the better the effect of acupuncture combined therapy intervention to improve nerve function.;?4 within 60 days,the longer the duration of combined acupuncture treatment,the better the efficacy may be in improving the lower limb function of the patient,but the time of combined acupuncture treatment within 14 days to improve the patient's neurological function and upper limb function of the clinical efficacy is poor.
