The Study On TCM Syndromes And Treatment Regularity Of Neurodermatitis And Clinical Observation

Purpose This study aims to collect the literature of TCM Treatment for neurodermatitis,systematically summarizing the dialectical laws and clinical experience of various doctors about the disease's treatment with data mining methods,which can provide a reference for clinical work,scientific research,and teaching work.Another part is observing the clinical efficacy of professor Zhang Chijin in the treatment of neurodermatitis with Chinese herbal decoction to explore its action mechanism,demonstrate its safety and effectiveness,and offer new ideas and theoretical guidance for the treatment of this disease.Methods Study on the Syndromes of Neurodermatitis Collect clinical research literatures on the TCM treatment of neurodermatitis published in the China Biomedical Literature Database,CNKI,Wanfang,and Weipu Database from 1949.10 to 2020.1.There are 177 literatures consistent with the Inclusion and exclusion criteria,and 225 pieces of dialectical medication data in medical record database.Data pre-processing should be performed on the type of the original data,the prescription used,and the drug name before it is inputted into the database.Through the statistical report system of the "Traditional Chinese Medicine Assistance System V2.5",explored the frequency of neurodermatitis syndrome and medication information Statistics.Discover the common drugs and drug combinations for neurodermatitis treatment,the laws of each type of medication with the analysis of association rules,improved mutual information method,complex system entropy clustering,unsupervised drop-level clustering method to.Clinical observation All the patients were derived from the dermatology clinic of the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine(2019.2-2020.1).A total of 76 patients who met the criteria of this study were randomly divided into two groups for comparison.The treatment group 38 took self-prepared Shuganliaoxuantang decoction,other 38 patients in the control group were orally treated with levocitizine hydrochloride tablets.Collect clinical data in the form of questionnaires,then sort them into an Excel database,using SPSS 22.0 statistical software to perform statistical analysis on the data,and finally draw conclusions.Results Study on the Syndromes of Neurodermatitis 1.This study involved 234 drugs,the frequency of the first 19 drugs was more than 40 times on average,namely Angelica 122,Raw Dihuang 117,Licorice 109,Baixianpi 86,Tribulus terrestris 83,Fangfeng 75,Peony Skin 69,Chiba 68.Sophora flavescens 66,white pheasant 63,cicada 60,crustacean 57,chuanxiong 48,nepeta 48,whole scorpion 44,shouwu vine 43,yellow cricket 43,chaihu 42 and salvia 41.2.The distribution of the four gases of the drugs is mainly cold,warm and flat medicine,cold medicine(48.59%)> warmness(28.40%)> flatness(19.92%)> coolness(2.81%)> heat(0.27 %).The distribution of the five flavors is mainly bitter,sweet and spicy medicine,bitter medicine(33.85%)> sweet medicine(31.66%)> spicy medicine(24.78%)> salty medicine(4.79%)> sour medicine(3.55%)> Astringent medicine(1.37%).Liver meridian is the main distribution of meridian.3.The types of commonly used drugs are heat-clearing drugs,tonic-deficiency drugs,anti-drug-extracting drugs,Pingganxifeng drugs,blood-activating and blood-stasis drugs,diuretic and damp-wet drugs,and tranquilizers.4.Of the 225 prescriptions included in the study for the treatment of neurodermatitis,97 are the most self-prepared,accounting for 43%.5.Through the analysis of the formula of the 225 prescriptions,while the support rate is 10%,the formula is shown in the network view 16,including raw radix rehmanniae,angelica,licorice,tribulus terrestris,white fresh skin,windproof,peony skin,and ground skin.Seeds,Sophora flavescens,Gardenia jasminoides,Red cockroaches,Paeonia lactiflora,Cicada spp.,Nepeta,Bupleurum,Chuanxiong,Whole scorpion,Polygonum multiflorum,Black-snake snake,Salvia miltiorrhiza,Silkworm,Rehmannia glutinosa,Heliotrope Winter and Sour Jujube Kernel.when the support is 25%,4 core drug pairs can be clearly identified from Figure 16: Raw Rehmannia and Angelica,Licorice and Angelica,Raw Rehmannia and Licorice,Angelica and Tribulus Terrestris.6.The most common types of neurodermatitis are blood deficiency,wind dryness syndrome,rheumatic fever syndrome,blood heat and wind syndrome,liver stagnation and fire syndrome,blood stasis syndrome,yin deficiency syndrome,rheumatism skin syndrome,etc.(1)The commonly used medicines for deficiency of wind and dryness are Angelica sinensis,Paeonia lactiflora,Rehmannia glutinosa,Radix Rehmannia,Radix Scutellariae,Ligusticum chuanxiong,Baixianpi,Tribulus terrestris,Fung,Shouwu rattan,Heterophyllum officinalis,Licorice.(2)The commonly used medicine for rheumatic fever is bitter Ginseng,white fresh skin,cork,radix rhizome,red peony,peony skin,cicada,windbreak,licorice.(3)The commonly used medicines for bloodheat fengsheng are radix rehmanniae,peony skin,red peony,gardenia,scutellaria baicalensis,flavescens,white fresh skin,Radix rehmanniae,angelica,licorice.(4)Liver stagnation and fire syndrome commonly used medicines are Chaihu,peony skin,gentian,gardenia,scutellaria baicalensis,angelica,white radix rhizoma,radix rhizoma glutinosa,Radix Polygoni,Scutellaria barbata,white fresh skin Licorice.(5)common drugs for blood stasis syndrome are angelica,peach kernel,safflower,Chuanxiong,Salvia miltiorrhiza,black-snake snake,Bupleurum chinensis,white fresh skin,silkworm,windbreak,Rehmannia glutinosa,Radix Scutellaria baicalensis,Rehmannia glutinosa,Poria,jujube kernel,licorice.(6)Yin deficiency certificate,which is often seen with blood deficiency syndrome,commonly used medicines are Rehmannia glutinosa,Ophiopogon japonicus,Paeonia lactiflora,and Rehmannia glutinosa Angelica,red peony,peony skin,black ginseng,white fresh skin,licorice.(7)Commonly used medicines for rheumatism skin card are earth skin,white fresh skin,Sophora flavescens,whole scorpion,cicada,windproof,tribulus terrestris,angelica,Shouwu Rattan and licorice.7.Based on the improved mutual information method and the entropy clustering algorithm for complex systems,a total of 15 new square combinations were obtained.Clinical observation After 8 weeks of treatment,the overall efficacy of the treatment group was compared: the effective rate of the treatment group was 100%,and the effective rate of the control group was 94.7%.The curative effect of the two groups was statistically significant(P <0.05).In the control group,the comparison was statistically significant(P <0.05),indicating that the clinical effect of the treatment group was better than that of the control group.The comparison of the scores of each condition was not statistically significant between the two groups(P> 0.05).The severity of skin lesions,the area of the lesions,and the symptom score were lower in the treatment group than in the control group,which was statistically significant(P < 0.05).The comparison of the scores of the disease degree before and after treatment in the two groups was lower than that before treatment,and there was a significant statistical significance(P = 0 <0.01).This shows that both therapies are effective in treating this disease.In terms of improving pruritus,the therapeutic effect was equivalent,and the treatment group was better than the control group in alleviating the severity of skin lesions,the area of the lesions,and the accompanying symptoms.Conclusion Study on the Syndromes of Neurodermatitis Based on association rule analysis of drug combination and analysis of unsupervised data mining technology,it is concluded that the drugs used for treating neurodermatitis are mostly cold,warm,flat and bitter,sweet,spicy.The main meridian is the liver meridian,and the efficacy is clearing heat,tonicity,and expelling wind.The core medicines are radix rehmanniae and angelica,licorice root and angelica,radix rehmanniae and licorice root,angelica and tribulus terrestris.This study also summarizes the common syndrome types of neurodermatitis and the rules of medication of each syndrome type,discovers several new prescriptions,which are useful for further clinical research and drug development,providing them new ideas and research directions.Clinical observation The self-made Shuganjieyutang decoction has obvious clinical effects in the treatment of neurodermatitis.It is especially good at reducing or even eliminating the symptoms of insomnia,anxiety,irritability,attention disorder,sorrow or sorrow,and improving the quality of patients' life.It has a low recurrence rate,and has obvious advantages.It is worthy for clinical promotion and use.
