The Mediating Role Of Cognitive Evaluation And Coping Style In Social Support And Post-traumatic Growth Of Breast Cancer Patients

Object This study based on the theory of psychological stress factors more model,describes the cognitive evaluation in patients with breast cancer,post-traumatic growth,coping styles and social support of the status quo,analysis of the relationship between them and the mechanism of action,to explore the cognitive appraisal and coping styles in the social support and the chain multiple mediation role of post-traumatic growth,in order to further build the breast cancer patients with positive cognitive intervention model provides the basis.Methods The breast cancer patients admitted from July 2016 to June 2017 were selected in two first-class hospitals in anhui province by cross-sectional investigation and convenient sampling.The survey was conducted through the general data questionnaire,the Cognitive Appraisal of Health Scale,the Post-traumatic Growth Inventory,the Medical Coping Modes Questionnaire,the Revised Life Orientation Test,and the Perceived Social Support Scale.Using EpiData3.0 software to establish database and SPSS22.0 software for statistical analysis of data,using MPLUS6.0 to build structural equation model;the statistical methods used are descriptive statistics,single factor ANOVA,chi-square test,t test,multiple linear regression analysis,Pearson correlation analysis,Bootstrap mediation effect analysis and so on.Results(1)The total score of cognitive evaluation for breast cancer patients was(68.65±6.42),and the scores of harmful/loss,benign/unrelated,challenge and threatdimensions were(23.46±5.89)points,(10.40±3.11),(20.79±3.92)and(14.00±3.76)points,total post-traumatic growth scores(62.73±15.03)points,of which the appreciation of life,relationship with others,new possibilities,personal strength,mental change scores were respectively(9.39± 2.47)points,(19.55±4.79)points,(15.40±4.40)points,(11.73±3.26)points,(6.66±2.18)points,the response mode score was(21.02±3.75)points,and the avoidance coping style score was(17.06±2.01)points,the score of yield response was(9.62±2.98)points,the total score of social support was(64.77±10.20)points,and optimistic tendencies total scores(12.45±2.02)points;(2)Univariate analysis showed that the scores of postoperative cognitive evaluation of breast cancer patients in different places of residence,payment methods and pathological types were statistically significant(P<0.05);the post-traumatic growth level of breast cancer patients was There were significant differences in scores between different places of residence,education level,income level,payment method,different caregivers,and other physical diseases(P<0.05);(3)Further multivariate regression analysis showed that pathological type,yielding coping style,and social support eventually entered the regression equation,which explained 52.5% of the total variation of health cognitive evaluation;cognitive evaluation,optimistic tendency,coping style,avoidance Coping style,yielding coping style,and social support eventually entered the regression equation,explaining a total of 54.1% of the total post-traumatic growth variability;(4)Pearson correlation analysis showed that cognitive evaluation was positively correlated with the mode of yielding coping(r=0.330,P<0.01),and negatively correlated with post-traumatic growth,coping style,avoidance coping style,and social support(r=-0.232,-0.206,-0.279,-0.176,P<0.05).Post-traumatic growth was positively correlated with coping style,avoidance coping style,social support and optimistic tendencies(r=0.592,0.413,0.504,0.196,P<0.01),and negatively correlated with yielding coping style(r=-0.581,P<0.01);(5)Coping had a mediating effect between social support and post-traumatic growth,witha mediating effect value of 0.267 and an effect amount of 37.0%;while cognitive evaluation and coping had the chain of multiple mediating effect between social support and post-traumatic growth,with a mediating effect value of 0.289 and an effect amount of 38.9%.Conclusion Patients with breast cancer had moderate levels cognitive evaluation and post-traumatic growth.Pathological type,yield response,and social support maybe the main influencing factors in cognitive evaluation of breast cancer patients.Cognitive evaluation,optimistic tendency,coping style,avoiding coping style,yielding coping style,and social support may be the main influencing factors of post-traumatic growth of breast cancer patients.There was significant correlation between cognitive evaluation,coping style,social support and post-traumatic growth in breast cancer patients.Cognitive evaluation and coping styles play a chain of multiple mediators between social support and post-traumatic growth.
