?Objection? To construct a targeted and effective hope nursing intervention program based on Snyder theory.To explore its application effect on improving the hope level,medical coping style and life quality of patients with acute leukemia chemotherapy.In order to provide reference for the intervention research related to hope nursing in China.?Methods? 1.Snyder theory based on acute leukemia chemotherapy patients hope to nursing intervention program construction.Through literature review and the study of hope theory,the first draft of the intervention program was formed.Then,the expert consultation was carried out,and 10 patients with acute leukemia chemotherapy were selected for pre-test study to form the final intervention plan.2.To evaluate the effect of Snyder theory-based nursing intervention on acute leukemia chemotherapy.This research is a quasi-experimental study,acute leukemia patients who met the inclusion criteria of this study were selected from the hematological department,Nanjing Drum Tower Hospital.The intervention was conducted in chronological order,and the intervention group was given hope care based on Snyder theory,which lasted for 4 weeks,twice a week,for 30 to 45 minutes each time,the control group received routine care in the hematology department.Data were collected in the follow time points: pre-intervention,postintervention,1 and 3 months post intervention.The evaluation tools were herth hope index,medical coping modes questionnaire and functional assessment cancer therapy-leukemia to evaluate the intervention effect.3.SPSS software version 23.0 for statistical analysis.Using rate,frequency,and percentage to describe the demographic data;using mean,standard deviation(?±s)to describe quantitative data;using generalized estimation equation to analyze the trend of hope level,medical coping style and quality of life score over time at each time point.All the tests showed significant difference with P<0.05.?Results?1.A total of 128 cases of acute leukemia chemotherapy patients were included in this study.During the intervention,2 cases were lost in the intervention group and 4 cases in the control group.A total of 6 cases were lost,with a loss rate of 4.7%,62 and 60 in the intervention group and control group.The age of the intervention group was(42.74±14.09)years,and that of the control group was(41.02±13.73)years.2.The changes of hope level total scores and the scores of each item in the preintervention,post-intervention,1 and 3 months post intervention were analyzed by using generalized estimation equation.The results showed statistically significant differences in hope level total scores,temporality and future,positive readiness and expectancy inter-group effects,time effects and interaction effects(P<0.05).There was no significant differences in interconnectedness between groups(P>0.05).There was a statistically significant differences in time effect and interaction(P<0.01).3.The changes of medical coping styles in the pre-intervention,post-intervention,1 and 3 months post intervention were analyzed by using generalized estimation equation.The results showed statistically significant differences in face dimension,avoidance dimension,yield dimension inter-group effects,time effects and interaction effects(P<0.05).4.The changes of life quality total scores and the scores of each item in the preintervention,post-intervention,1 and 3 months post intervention were analyzed by using generalized estimation equation.The results showed statistically significant differences in life quality total scores,physical well-being,social/family well-being,emotional well-being,functional well-being inter-group effects,time effects and interaction effects(P<0.05).There was no significant differences in leukemia subscale between groups(P>0.05).There was a statistically significant differences in time effect and interaction(P<0.01).?Conclusion?Compared with routine nursing,Snyder theory-based intervention program of hope nursing for patients with acute leukemia chemotherapy can effectively improve patients' hope level,promote positive change of medical coping style,and improve quality of life.The intervention program has good maneuverability,pertinence and extensibility.
