Objective: mechanical ventilation(MV)can provide the necessary alveolar ventilation for patients with respiratory failure,eliminate excessive carbon dioxide accumulated in the body,significantly improve lung ventilation function,improve lung compliance,reduce respiratory muscle work,alleviate ventilator fatigue,and is conducive to the recovery of respiratory function,so as to improve the lung function of patients.Among them,the efficacy of noninvasive positive pressure ventilation(NPPV)in patients with respiratory failure and the protection of vehicular ventilator in the transport of critically ill patients have been clinically confirmed.The purpose of this study was to evaluate the clinical efficacy of ultra early respiratory support therapy for patients with acute heart failure complicated with hypoxemia during pre-hospital emergency treatment.Methods: 60 patients with acute force failure complicated with hypoxemia admitted to our emergency center from January 2019 to March 2020 were selected and divided into two groups according to the principle of whether to use vehicle ventilator,including 27 cases in the control group(group A)and 33 cases in the observation group(group B).All patients were given symptomatic treatment combined with specific conditions.Patients in group A were supplemented with ordinary oxygen supply,and group B was assisted with vehicle ventilator for auxiliary ventilation.The vital signs of the patients were monitored continuously and dynamically by the on-board multi parameter monitor,and the vital signs values were recorded at the time of pre-hospital treatment and arrival at the hospital,The improvement effect of clinical symptoms,vital signs,pro-BNP II,blood gas analysis,hospitalization days and mortality of the two groups were compared before and after treatment.Results: After active symptomatic treatment,the vital signs,pro-BNP II and clinical symptoms of the two groups were improved.The recovery of vital signs,pro-BNP II and blood gas analysis indexes in group B was significantly better than that in the control group.The total effective rate of treatment was significantly increased,and the length of stay and mortality were lower than those in the control group(P < 0.05).Conclusion: Conventional drugs and common nasal catheter oxygen supply have certain therapeutic effect on patients with acute heart failure and hypoxemia.In the pre-hospital ultra early use of vehicle ventilator auxiliary ventilation treatment can more effectively improve the clinical symptoms of patients,reduce the patient's hypoxia,is conducive to the recovery of lung function,at the same time,it has a certain role in reducing the length of hospital stay and mortality,which significantly improves the treatment effect,has important clinical significance and certain economic value,and is worthy of active promotion and application in prehospital emergency.
