Objectives To understanding whether early chest physical therapy can reduce the incidence of ventilator-associated pneumonia in patients with invasive mechanical ventilation Early chest physiotherapy,invasive mechanical ventilation.Mathods 117 adult patients who were admitted to the ICU for the first time and had mechanical ventilation of ?48 hours were randomly divided into two groups,named study group and control group,59 cases and 58 cases respectively.The patients in the control group were given conventional drug treatment and nursing,and the patients in the experimental group were given an individualized chest physical therapy program.The clinical pulmonary infection score(CPIS),acute physiology and chronic health status score II(APACHE II),and oxygen utilization were evaluated the time patients admitted to the ICU and leaving ICU,respectively.The primary outcome was the incidence of VAP in both groups.Secondary outcomes were mortality,MV duration,length of ICU duration,and total length of hospital duration.Results At the end of treatment,the CPIS score,APACHE II score,oxygenation index,and blood oxygen saturation(Sp O2)were improved in both groups.The improvement in the experimental group was more obvious than in the control group(P<0.05).Compared with the two groups,the incidence of VAP was lower in the experimental group,the VAP appeared later,the MV duration was shorter,the offline success rate was higher,the total hospital stay and ICU stay time were shorter,and the difference was statistically significant(P<0.05).The mortality of the control group was significantly higher than that of the experimental group after one month of discharge from the two groups.Conclusions Early chest physical therapy can reduce the incidence of VAP,and can reduce hospitalization time,ICU stay time,MV duration,improve survival rate and overall prognosis;Early chest physical therapy is a safe and effective treatment in ICU.
