Correlation Analysis Of Serum Irisin Level And Carotid Atherosclerosis In Type 2 Diabetes Patients

PurposeType 2 diabetes(T2DM)is one of the chronic metabolic diseases affecting human health.Its incidence is increasing year by year.Its chronic complications are important factors affecting the quality of life and death.Atherosclerosis(AS)is not only an important macrovascular complication in the progression of T2 DM,but also one of the important causes of death of obesity and cardio-cerebrovascular diseases.At present,the pathogenesis of macrovascular complications of T2 DM is still unclear and effective treatments are relatively limited.In recent years,irisin is a popular muscle factor produced mainly by skeletal muscles.It is mainly produced by exercise,which is an important basic treatment for many chronic metabolic diseases.This article intends to study the association between irisin and T2 DM patients with carotid atherosclerosis(CAS).MethodsWe selected a total of 56 patients diagnosed with type 2 diabetes who were hospitalized in the Endocrinology Department of the First Affiliated Hospital of Nanchang University from August to December in 2019.According to the carotid artery intima-media thickness(CIMT),the patients were divided into two groups:T2DM without CAS group(n = 25)and T2 DM with CAS group(n = 31).At the same time,30 healthy people matched age and gender in the physical examination center of our hospital were selected as normal control(NC)group.While Collectting the baseline data of age,gender,blood pressure(BP),body mass index(BMI)and course of the disease,we detected fasting plasma glucose(FPG),triglycerides(TG),cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),urea nitrogen(BUN),uric acid(UC),creatinine(CR),fasting cpeptide(CP)and hypersensitive c-reactive protein(hs-CRP).The levers of irisin in serum was detected by enzyme-linked immunosorbent assay(ELISA).Results(1)There was no statistically significant difference of age,gender,course of the disease and the level of BMI,TG,TC,Cr,UA,CP between the three groups(P>0.05).(2)Compared with NC group,the SBP,DBP and the level of BUN,FPG,hs-CRP were increased(P<0.05)and the level of HDL-C were decreased(P< 0.01)in T2 DM with CAS group;SBP,DBP and the level of FPG,hs-CRP were increased(P<0.05)and the level of HDL-C,LDL-C were decreased(P<0.05)in T2 DM without CAS group.In contrast to T2 DM with CAS group,the SBP,DBP and the level of FPG,HDL-C,LDL-C,hs-CRP,BUN in T2 DM without CAS had no statistically significant difference(P>0.05).(3)The serum irisin levels had no statistically significant difference between NC group with T2 DM group(including the T2 DM with CAS and without CAS group)(P>0.05).The serum irisin levels of T2 DM with CAS group were significantly lower than NC group(P<0.05).(4)Spearman correlation analysis showed that there was a significant negative correlation between the level of irisin and development of CAS in T2 DM patients.Pearson correlation analysis showed that there was a significant negative correlation between the level of irisin and SBP,DBP.The level of hs-CRP was positively correlated with the level of TG,BMI and CP,and negatively correlated with the level of HDL-C.(5)The occurrance of CAS(1 = yes,0 = no)was taken as the dependent variable and age,irisin,SBP,DBP,LDL-C,HDL-C,FPG,BUN as independent variables.Binary logistic regression analysis showed that Irisin and HDL-C were the independent protective factors of CAS(OR=0.153,P=0.017;OR=0.068,P=0.031),While Age and FPG were independent risk factors of CAS(OR=1.248,P=0.001;OR=1.327,P=0.005).Conclusion(1)The levels of serum irisin in T2 DM patients with CAS were significantly lower than that in NC group,and were lower than that T2 DM patients without CAS.There was no significant difference in serum irisin levels between patients with T2 DM and NC group.It indicated that irisin may be related to the development ofT2 DM and carotid atherosclerosis.(2)Correlation analysis showed that there was a significant negative correlation between serum irisin level and the occurrence of CAS in T2 DM patients.Serum irisin level was significantly negatively correlated with SBP and DBP.Logistic regression analysis showed that irisin was an independent protective factor for CAS.It suggested that irisin may slow down or inhibit the devepment of macrovascular complications in T2 DM patients and also play a protective role in cardiovascular diseases.(3)Compared with NC group,the serum hs-CRP level of T2 DM patients was significantly higher.Correlation analysis showed that the level of hs-CRP was positively correlated with the level of TG,BMI and CP,and negatively correlated with the level of HDL-C.It suggested inflammation affected chronic metabolic diseases.It was speculated that irisin may have a protective effect on chronic metabolic diseases by reducing inflammatory response.
