Objective:To observe the effect of metformin on serum Irisin level its role in delaying atherosclerosis(AS)in newly diagnosed type 2 diabetes mellitus(T2DM)patients.Methods:120 patients with newly diagnosed Type 2 diabetes mellitus were enrolled and divided into two groups randomly: Low dose group(60 subjects)were treated with metformin(250mg,tid)for 6 months,while High dose group(60 subjects)were treated with metformin(500mg,tid)for 6 months.Before and after treatment,serum Irisin level,CIMT,serum MMP9 level,BMI,WHR,lipids,plasma glucose,insulin resistance parameters and other biochemical indexes were measured.Results:Before treatment,there was no significant difference in baseline data between the low dose group and high dose group(P > 0.05).Compared with prior treatment,the level of serum Irisin in low dose group increased significantly [(215.17±89.96)vs(356.66±102.32),P<0.05].Decreased CIMT[(1.43±0.89)vs(1.10±1.21)]and increased irisin level [(238.17±67.96)vs(389.43±93.43),P<0.05]in high dose group was observed.The levels of MMP-9 and oxidative stress in both groups decreased significantly(P < 0.05).Compared with the low dose group,the decrease of MMP9 and MDA and the increase of serum Irisin was more obvious in the high dose group(P < 0.05).Correlation analysis showed that after 6 months,?CIMT in high dose group was negatively correlated with ?Irisin,and positively correlated with ?BMI,?FPG,?TG and changes of oxidative stress level.?MMP9 was negatively correlated with ?Irisin and positively correlated with ?FPG and changes of oxidative stress level.?Irisin was negatively correlated with the change of oxidative stress level.Furthermore,?CIMT and ?MMP9 in high dose group were used as dependent variables,other related factors were used as independent variables to carry out multivariate linear stepwise regression analysis.The results showed that ?Irisin and changes of oxidative stress level were independent influencing factors of ?CIMT and ?MMP9.Conclusion:Metformin can delay the progression of carotid atherosclerosis in newly diagnosed patients with type 2 diabetes mellitus,partly due to the elevated of Irisin level and the improvement of oxidative stress.
