Purpose:To explore the clinical effect of acetylcysteine combined with low molecular weight heparin in the treatment of interstitial pneumonia by comparing the changes of pulmonary function,blood gas analysis and chest imaging before and after treatment based on the self assessment of clinical symptoms in patients with interstitial pneumonia.Method:According to the diagnostic criteria of interstitial pneumonia,60 patients in the respiratory department and rheumatology and immunology department of The First Affiliated Hospital of Nanchang University from October 2017 to December 2019 were selected and randomly divided into an observation group and a routine group,with the same number of patients in each group(30 cases).This study was retrospectively analyzed.According to the conditions of patients with interstitial pneumonia,the therapeutic schemes,such as oxygen inhalation,cough relief and sputum eliminating,spasm and asthma relief,and anti-inflammatory effect,were carried out for the routine group for three months;on the treatment basis of the routine group,the combined treatment of acetylcysteine and low molecular weight heparin was performed for the observation group with the same treatment course.Then the clinical symptoms,pulmonary function test(PET)indexes,arterial blood gas(ABG)analysis indexes and HRCT of the patients with interstitial pneumonia before and after treatment were analyzed,so as to summarize the clinical effect of acetylcysteine combined with low molecular weight heparin in the treatment of interstitial pneumonia.Result:1.Basic data and clinical features: before the treatment,no statistical difference was showed in the basic data and clinical features between the two groups(P > 0.05);after the treatment,the self-reported score of the clinical features(cough,coughing of sputum,breathing difficulties,Velcro rale in lung)in the routine group was lower than that before the treatment,but not statistical significance was showed(P > 0.05);while the self-reported score of the clinical features in the observation group was significantly reduced(P < 0.05).It could be seen from the self-reported scores of the clinical features of patients in the two groups that the self-reported score of the observation group is significantly lower than that of the routine group,with statistical significance(P < 0.05).2.Pulmonary function indexes: after the treatment,the PET indexes of the routine group were higher than that before the treatment,without statistical significance(P > 0.05),while the PET indexes of the observation group were increased,wherein the increases of VC%pred and DLCO/VA%pred were significant,with statistical significance(P < 0.05);compared with those in the routine group,the PET indexes in the observation group were significantly improved,and statistical significance was showed(P < 0.05).3.ABG analysis indexes: after the treatment,increases of PO2 were showed in the two groups;the differences in PO2 and PCO2 for the routine group before and after the treatment were not significant(P > 0.05);while in the observation group,the PO2 was significantly increased and the PCO2 was significantly reduced after the treatment,with statistical significance(P < 0.05);compared with those in the routine group,the PO2 and PCO2 were both improved,especially the reduction of the PCO2,with the statistical significance(P < 0.05).4.Chest HRCT: there was no statistically significant difference in honeycomb shadow in chest HRCT performance of the observation group compared with before treatment after treatment(P > 0.05),and other three chest HRCT manifestations were significantly reduced after the combined treatment,with statistical significance(P < 0.05);By comparing the four manifestations of HRCT in the routine group before and after the treatment,no statistical significance was showed(P > 0.05).5.Effective rate: after the treatment,the clinical treatment was effective for 28 patients in the observation group,accounting for 93.33% of the group,but the effective rate in the routine group was only 70.00%.The statistical difference was showed in the total effective rate of the clinical treatment between the two groups(P < 0.05).6.The occurrence of adverse reactions: after the treatment,there was one patient with thrombocytopenia,two patients with nausea and vomiting and two patients with diarrhea in the observation group,while in the routine group,there were three patients with nausea and vomiting and one patient with diarrhea,who were relieved after symptomatic treatment;and no other significant adverse reactions were showed.By comparing the adverse reactions of the two groups,no statistical significance was showed(P > 0.05).Conclusion:By the combined treatment of acetylcysteine and low molecular weight heparin for patients with interstitial pneumonia,the clinical features,such as cough,breathing difficulties and Velcro rale in lung can be reduced,the quality of life of patients in the later period is improved,the clinical treatment effect is good,and no obvious adverse drug reactions are showed,indicating that the proposed combined treatment has a higher significance of clinical promotion.
