Objective:1.To investigate the relationship between Chemokine ligand 5(CCL5)and periodontal index,glucose metabolism and inflammatory factors in patients with type2 diabetes mellitus(T2DM)and periodontitis.2.To determine the effect of initial periodontal therapy on periodontal index,inflammation control and serum CCL5 in type 2 diabetic patients with periodontitis.Methods:1.40 cases of T2 DM with periodontitis and 40 cases of periodontitis were selected.The general clinical data of patients were collected.The periodontal state of patients was examined.The peripheral blood glucose(FPG),HbA1 c,TNF-?,IL-6 and CCL5 were measured and compared.Pearson correlation was used to analyze the correlation between CCL5 level and clinical indexes in T2 DM patients with periodontitis.2.40 T2 DM patients with periodontitis were divided into control group and initial periodontal therapy group according to whether to receive initial periodontal therapy or not.The control group only received oral health education,while the initial periodontal therapy group received initial periodontal treatment intervention.Three months later,the periodontal status of the patients was examined.FPG,HbA1 c,TNF-?,IL-6 and CCL5 were measured and compared.Results:1.Compared with periodontitis patients,the periodontal inflammation of T2 DM patients with periodontitis was higher,FPG,HbA1 c,TNF-?,IL-6 and CCL5 were significantly higher(P < 0.05).2.There was a significant positive correlation between CCL5 and periodontal index,FPG,HbA1 c,TNF-? and IL-6 in T2 DM patients with periodontitis(P < 0.05).3.Compared with the baseline value,the periodontal index of the patientsreceiving initial periodontal therapy decreased significantly after 3 months(P < 0.05),but there was no significant change in the control group(P > 0.05).Compared with the control group,the periodontal index of the patients receiving initial periodontal therapy also decreased significantly after 3 months(P < 0.05).4.Compared with the baseline value,the values of FPG and HbA1 c of the patients receiving initial periodontal therapy decreased significantly after 3 months(P< 0.05),but there was no significant change in the control group(P > 0.05).Compared with the control group,the values of FPG and HbA1 c of the patients receiving initial periodontal therapy also decreased significantly after 3 months(P <0.05).5.Compared with the baseline value,the values of TNF-? and IL-6 of the patients receiving initial periodontal therapy decreased significantly after 3 months(P< 0.05),but there was no significant change in the control group(P > 0.05).Compared with the control group,the values of TNF-? and IL-6 of the patients receiving initial periodontal therapy also decreased significantly after 3 months(P <0.05).6.Compared with the baseline value,the value of CCL5 of the patients receiving initial periodontal therapy decreased significantly after 3 months(P < 0.05),but there was no significant change in the control group(P > 0.05).Compared with the control group,the value of CCL5 of the patients receiving initial periodontal therapy also decreased significantly after 3 months(P < 0.05).Conclusion:1.CCL5 may play an important role in the interaction between T2 DM and periodontitis,which may be a potential clinical indicator to evaluate the progress and prognosis of T2 DM patients with periodontitis.2.Initial periodontal therapy can improve the periodontal symptoms and regulate the expression of inflammatory factors in T2 DM patients with periodontitis,and its mechanism may be related to the regulation of CCL5 expression and release.
