Objective:To explore the correlation between H-type hypertension and carotid atherosclerotic plaque in patients with acute cerebral infarction,So as to provide practical scientific basis for the prevention and treatment of acute cerebral infarction.Methods:340 patients with acute cerebral infarction were collected from june 2018 june 2019 in the first affiliated Hospital of Shihezi University medical School.We collected the gender,age,hypertension history,diabetes history,smoking history and drinking history of all the patients;and the laboratory indexes of homocysteine,blood lipid and so on were recorded in detail.According to the results of carotid artery color ultrasound(carotid intimal medial thickness),it was divided into plaque group and control group.According to hypertension and Hcy level,they were divided into H hypertensive group,pure hypertension group,HHcy group,and normal group.all the data were statistically analyzed.Results:1.Geometric mean of serum homocycteine was 25.59+14.65?mol/L in males and20.47+11.89?mol/L in females(P<0.05),The Hcy levels in the males were higher than those in the females in each group(P<0.05)and the levels of Hcy increased with age in both groups.Single factor analysis of general data and laboratory indexes in plaque group and control group:hypertension,diabetes history,H-type hypertension,drinking histories,age,UA,Cr,Hcy,Cystatin C,between the two groups have significant difference(P<0.05).the diferences of other risk factors were not obvious;The multifactor Logistic reession analysis showed that Hcy OR=1.382),95%CI(1.242-1.538);hypertension(OR=4.070),95%CI(1.698-9.756)and H-type hypertension(OR=5.248),95%CI(0.053-0.680)are risk factors of carotid atherosclerotic plaque formation in patients with ACI,and the regression coefficient?(1.658)of H-type hypertension was higher than that of hypertension(1.404)and Hcy(0.323).3.The detection rate of four subgroups in plaque group was:the detection rate of H hypertensive group(56.11%)>HHcy group(29.54%)>pure hypertension group(12.24%)>normal group(2.11%).HHcy group 71 cases,significantly higher than the pure hypertension group(?~2=25.913,P<0.05)and the normal group(?~2=78.874,P<0.05).H-type hypertension totaling135 cases,significantly higher than the pure hypertension group(?~2=49.633,P<0.01)and the normal group(?~2=119.057,P<0.05).H hypertensive groupp and tHHcy group?the pure hypertension grou and the normal group,there was no significant difference(P<0.05).4.Analysis of interaction between hypertension and HHcy on carotid atherosclerotic plaque formation in ACI patients showed that they had positive interaction(S=1.43).Conclusions1.The plasma Hcy levels are obviously different in acute cerebral infarction patients according to the sex and age:the Hcy levels in the males are higher than those in the females,and increases gradually with age.2.Hcy?hypertension?H-type hypertension are risk factors of carotid atherosclerotic plaque formation in ACI patients.3.Hcy and HHcy have synergistic effect on carotid atherosclerotic plaque formation in ACI patients...
