Objectives: The purpose of this study was to construct a fall risk evaluation index system for cardiovascular disease patients in accordance with the characteristics of the specialty,and to analyze and test the reliability and validity of the index system.It provides an evaluation tool for clinical nurses to evaluate the risk of falls of cardiovascular patients in hospital,identify the risk of falls as early as possible,and take preventive measures to reduce the incidence of falls.Methods:1.Meta-analysis and semi-structured interviews were used to preliminarily determine fall risk assessment indicators for patients with cardiovascular disease.2.After two rounds of expert letter consultation,the index items were further screened and improved,and the weight of the evaluation index system was determined through analytic hierarchy process.Finally,the fall risk evaluation index system for patients with cardiovascular disease was formed.3.The convenience sampling method was adopted to select 197 patients from the department of cardiology of three first-class hospitals in Lanzhou City as the investigation objects to test the reliability and validity of the indicators.Results:1.Risk factor determination results1.1 Meta analysis results: The influence factors of the fall in patients with cardiovascular disease(CVD)Meta analysis,the results showed that age,taking medicines for cardiovascular disease,daily life ability is poor,visual impairment,fear of the fall,gait instability,need auxiliary tool,and there are always fall history of these eight factors are the factors affecting cardiovascular patients at risk of falling.1.2 Results of qualitative interviews: through interviews with 3 cardiovascular head nurses and 9 nurses,we obtained 5 topics related to cardiovascular patients' falls: patient factors,disease factors,medical factors,environmental factors,and management factors.2.Result of index system constructionThe recovery rate of the first round of questionnaire was 100% and the effective rate was 96%.The recovery rate and effective rate of the second round of questio nnaire were 100%,indicating that the experts had a high degree of cooperation.In the results of the two rounds of consultation,the expert authority coefficient was higher than 0.7,indicating that the expert authority was higher.Through two rounds of consultation by mail,the expert opinions are in agreement.The final fall risk evaluation index system for patients with cardiovascular disease includes six first-level indicators,18 second-level indicators and 52 third-level indicators.The index weight was determined by the analytic hierarchy process(AHP),in which the weight of disease factors was the highest(0.4092),and the index consistency ratio CR at all levels was less than 0.10.3.Reliability and validity test results of the indicator systemThe reliability and validity test of the index system was carried out by issuing questionnaires.After statistical test,the total Cronbach's of the evaluation index of the system was 0.751,indicating a good reliability.In this study,I-CVI was 0.86~1and S-CVI was 0.86,indicating that the content of each item was in good agreement with the measured content.A total of 9 common factors were extracted by exploratory factor analysis,and the contribution rate of cumulative variance was 66.591%.Conclusions:In this study,risk factors for falls of cardiovascular patients were obtained through meta-analysis and qualitative interviews.Then,the risk assessment index syste m for falls of patients with cardiovascular diseases was constructed through Delphi e xpert consultation method and analytic hierarchy process.Finally,the reliability and validity of the index system were tested by issuing questionnaires.Through statistical analysis,it is concluded that the setting of items is scientific and reliable,the weight setting of indicators is appropriate and accurate,and the system has a good reliab ility and validity,which can be used to evaluate the risk of falls of hospitalized pat-ients with cardiovascular disease,providing a scientific basis for nursing staff to take corresponding preventive measures in time.
