Objective To analyze the safety,short-term efficacy,advantages and difficulties of early cervical cancer hysterectomy with robotic suspension combined with transvaginal closure of cervical tumor.Methods The clinical data of the patients who underwent the first robotic surgery in the first affiliated hospital of Nanchang university for early cervical cancer from January 2018 to December 2019 were collected.149 patients were included in this study according to the inclusion and exclusion criteria previously designed.Among them,104 patients underwent extensive hysterectomy and pelvic lymph node dissection for cervical cancer by lifting uterine organ,and 45 patients underwent extensive hysterectomy for cervical tumor by robotic suspension combined with vaginal closure.Through the records to compare two groups of patients with height,weight,age,comorbidity,FIGO staging,and other general situation and intraoperative blood loss,sides near the palace resection length,length of vaginal removal,intraoperative complications and postoperative complications,blood transfusion rate,transit operation rate and postoperative pathology,short-term prognosis related information,to evaluate the early cervical cancer robot suspension combined transvaginal hysterectomy closed cervical tumor wide application safety and short-term curative effect.Results There was no statistical difference between the two groups.Intraoperatie blood loss no statistical difference between the groups,postoperative hospitalization days no statistical difference between the two groups,there was no difference in the two groups of patients with postoperative blood transfusion rate,ureter injury rate,the rate of major vascular injury,bladder injury rate,bowel injury rate of intraoperative complications such as no statistical difference,intestinal obstruction,infection,lower limb vein thrombosis and postoperative ureteral postoperative complication such as leakage,pulmonary embolism no statistical difference between the two groups,transit open rate is no statistical difference between the groups,postoperative pathological condition no statistical difference between the two groups,short-term disease-free survival and overall survival difference between the two groups.Conclusion(1)it is safe and feasible to perform extensive hysterectomy with robotic laparoscopic suspension combined with trans-vaginal closure of cervical tumors.It has the advantages of less bleeding,faster postoperative recovery,shorter hospitalization time and smaller abdominal incision in laparoscopic surgery.It can reduce the intraoperative tumor extrusion and diffusion,and is more consistent with the principle of non-tumor surgery.(2)lower pelvic infection rate after extensive hysterectomy with robotic laparoscopic suspension combined with trans-vaginal closure of cervical tumors.(3)the short-term efficacy of robotic laparoscopic suspension combined with trans-vaginal closure of cervical tumors in extensive hysterectomy is reliable,and the long-term efficacy needs to be verified by further follow-up.
