Objective:To observe the effect of enteral nutrition emulsion combined with probiotics on inflammatory factors and cardiac function in elderly patients with chronic heart failure(CHF)and malnutrition,and to provide experimental data for standardized nutritional support for CHF patients with malnutrition.Methods:90 elderly patients with CHF and malnutrition received in the first people's hospital of Jiujiang City were randomly divided into general feeding group,enteral nutrition group and enteral nutrition combined with probiotics group.All patients received CHF standard treatment,in which patients in general feeding group ate freely according to their wishes,the enteral nutrition group received enteral nutrition emulsion,the enteral nutrition combined with probiotics group added probiotics to the enteral nutrition group,before treatment,treatment for 30 days and 60 days,the index of cardiac function,6 minutes walking distance and nutritional evaluation were measured and compared and analyzed respectively.Results:1 There were no significant changes in LVEF,LVEDD and LVESD between groups at 30 d compared with before treatment(p>0.05).After 60 days of treatment,,the LVEF of patients in enteral nutrition group and enteral nutrition combined with probiotics group were statistically significant(p<0.05),and the decrease in LVEDD and LVESD was statistically different(p<0.05).However,there has no obvious difference between enteral nutrition group and enteral nutrition combined with probiotics group(p>0.05).2 The walking distance at 6 minutes was significantly increased in enteral nutrition group and enteral nutrition combined probiotics group at 30 days of treatment(p<0.05).At 60 days of treatment,the 6-minute walking distance in enteral nutrition group and enteral nutrition combined probiotics group was significantly increased compared with 30 days of treatment(p<0.05),but there has no obvious difference between the enteral nutrition group and the enteral nutrition combined with probiotics group(p>0.05).3 The NT-proBNP level of patients in enteral nutrition group and enteral nutrition combined probiotics group were significantly reduced at 30 days of treatment(p<0.05).At 60 days of treatment,the level of NT-proBNP in each group were significantly reduced compared with 30 days of treatment(p<0.05),the NT-proBNP levels of enteral nutrition group and enteral nutrition combined with probiotics group decreased further(p<0.05),the NT-proBNP level of the two groups was significantly lower than that of the general feeding group(p<0.05),there was no significant difference between the two groups in the same period(p>0.05).4 The levels of IL-6 and TNF-? in patients in enteral nutrition group and enteral nutrition combined probiotics group were significantly reduced at 30 days of treatment(p<0.05).The IL-6 and TNF-? levels of enteral nutrition group and enteral nutrition combined with probiotics group were significantly lower in 60 d compared with 30 d(p<0.05),and the levels of IL-6 and TNF-? in both groups were significantly lower than those in the general feeding group(p<0.05),There was no significant difference between the two groups in the same period(p>0.05).5 The BMI,TSF and AMC values of enteral nutrition group and enteral nutrition combined with probiotics group were significantly increased at 30 d?60d of treatment,and the proportion of patients with MNA-SF<8 points was significantly decreased((p<0.05).The BMI,TSF and AMC values were significantly higher in both groups after 60 days of treatment than in the general feeding group during the same period(p<0.05),the number of patients with MNA-SF<8 points was also significantly lower than that of the general feeding group during the same period(p<0.05),The relative indexes of enteral nutrition combined with probiotics group were significantly better than those of enteral nutrition group(p<0.05).6 The levels of TP,PA and ALB in enteral nutrition group and enteral nutrition combined probiotics group were significantly higher in the enteral nutrition group and the enteral nutrition combined with probiotics group after 30 days of treatment than before(p<0.05),and the levels of TP,PA and ALB in enteral nutrition group and enteral nutrition combined with probiotics group were increased at 60 d,compared with the general feeding group at 30 d and the same period(p<0.05),the related indexes of enteral nutrition combined with probiotics group improved the enteral nutrition group obviously(p<0.05).Conclusion:1 Enteral nutrition support therapy can improve the nutritional status,inflammatory cytokine,and cardiac function in elderly CHF patients with malnutr-ition,and with the extension of intervention time,the improvement effect is more significant.2 Enteral nutrition combined with probiotic adjuvant intervention in improving cardiac function and inflammation in elderly patients with CHF showed no significant difference compared with enteral nutrition alone.During the 60-day observation period,with the extension of the intervention time,enteral nutrition combined with probiotic adjuvant intervention Of patients had significantly better nutritional status than patients with only enteral nutrition intervention.
