Objective: This study was to analyze the correlation between Helicobacter pylori(Hp)infection and frailty by evaluating the frailty of hospitalized elderly patients with hypertension and detection of Hp,so as to provide a theoretical basis for further treatment.Methods: 227 patient with hypertension aged ? 65 with gastrointestinal symptoms(such as nausea,vomiting,belching,acid regurgitation,abdominal pain and abdominal distension)in the geriatrics department and cardiovascular medicine department of the first affiliated Hospital of Medical College of Shihezi University were included from October 2018 to October 2019.Frailty assessment used questionnaire survey according to the FRAIL scale and all inpatients was divided into three groups:nonfrail group(n=85),prefrail group(n=129)and frail group(n=63).At the same time,the Hp infection was evaluated according to the results of gastric mucosal biopsy,and the relevant data of inpatients were collected.Finally,the data was sorted respectively through multivariate logistic regression analysis to understand the correlation between Hp infection and frailty in hospitalized elderly patients with hypertension.Results:1.A total of 277 elderly inpatients with hypertension were included in this study,including 85 cases in nonfrail group,129 cases in prefrail group and 63 cases in frail group,with the frailty incidence of 22.74%.2.Analysis of the related factors of frailty in elderly inpatients with hypertension:(1)Comparison of social demographic data among nonfrail group,prefrail group and frail group: age,education degree,marriage,living style,body mass index(BMI)and the use of multiple drugs were significantly different among the three groups(P<0.05).(2)Comparison of blood routine and blood biochemical indexes: White blood cell count(WBC),albumin(ALB),serum creatinine(Scr),high-density lipoprotein cholesterol(HDL-C)and homocysteine(Hcy)were significantly different among the three groups(P<0.05).(3)Comparison of chronic diseases: stroke,coronary heart disease(CHD),diabetes and combination of multiple diseases were significant differences among the three groups(P<0.05).3.Correlation between Hp infection and frailty in elderly hypertensive inpatients: the Hp infection rate in elderly hypertensive patients included in the study was 30.21%.Univariate analysis showed that the infection rate of Hp in the prefrail group and the frail group was significantly higher than that in the nonfrail group,and there was significant difference among the three groups(P<0.05).Multivariate Logistic regression analysis showed that after correction of these above factors(age,the use of multiple drugs,combination of multiple diseases,WBC,ALB,Scr,HDL-C and Hcy),there were significant differences in Hp infection between nonfrail group and prefrail group(model I),nonfrail group and frail group(model II)(P<0.05).Conclusion:1.The frailty rate of hospitalized elderly patients with hypertension is 22.74%,suggesting that clinical doctors should pay attention to the frailty of elderly patients with hypertension and take targeted intervention measures to reduce the occurrence of frailty.2.Age,education degree,marriage,living style,BMI,multiple drugs,stroke,coronary heart disease,diabetes,coexistence of multiple diseases,WBC,Hb,ALB,Scr,TG,HDL-C and Hcy may be the influencing factors of frailty in hospitalized elderly patients with hypertension.3.Frailty is associated with Hp infection in elderly Hypertension patients.Hp infection may lead to frailty by affecting the levels of HDL-C and Hcy in patients.In view of this,clinicians should not only strengthen the assessment of frailty,but also attach great importance to the influence of Hp infection prevalence.
