Objective: Spontaneous cerebral hemorrhage(ICH)is a primary nontraumatic cerebral parenchymal hemorrhage,a subtype of poor prognosis of stroke,with a one-month mortality rate of nearly 40%,and 75% of patients with ICH die or have severe disabilities.Despite the rapid development of various diagnosis and treatment technologies,the decision of drug treatment after cerebral hemorrhage is still a clinical focus.Statins,common lipid-lowering drugs in the clinic,have neuroprotective effects and are widely used in the primary and secondary prevention of cardiovascular and cerebrovascular diseases.However,the effect of statins on the occurrence and outcome of spontaneous intracerebral hemorrhage is unclear,and the results of many clinical and basic studies are also divergent.This study conducted a meta-analysis of the effects of statins with and without statins on the prognosis of patients with spontaneous cerebral hemorrhage,in order to systematically evaluate the value of statins in patients with cerebral hemorrhage,in order to explore whether statins can improve the brain Prognosis of bleeding.methods: Computer searched domestic and foreign databases.Based on limited terms,high-quality randomized case-control studies of patients with cerebral hemorrhage taking statins published between December 2006 and December 2019 were performed in Chinese and English.The retrieved literatures were screened out,and two researchers independently performed quality evaluation and data extraction,and then performed meta-analysis on the extracted clinical data.Results:A total of 17 literatures were included in this study,with a sample size of 16,988,of which 3501 were in the statin group and 13487 were in the control group.Mortality,MRS score,and hematoma volume were used as indicators to evaluate the prognosis of cerebral hemorrhage.According to the Newcastle-Ottawa Scale(NOS)document quality evaluation scale,the score is(6 to 8 points),which indicates that the document quality is good.Meta-analysis of the clinical data extracted from the literature shows that compared with the non-statin group,the statin group reduced the total mortality after cerebral hemorrhage(P = 0.04)and 90-day mortality(P = 0.02);In terms of functional prognosis,the statin group improved the functional prognosis 90 days after intracerebral hemorrhage(P = 0.01).In terms of reducing the amount of hematoma in cerebral hemorrhage,there was no significant difference between the statin group and the non-statin group.Conclusions: 1.Statins can reduce the overall mortality after cerebral hemorrhage and improve the long-term(90d)survival rate.2.Statins have improved the prognosis of patients with cerebral hemorrhage,especially the long-term(90d)prognosis.3.Application of statins does not increase hematoma volume of cerebral hemorrhage.
