Objective:Analysis of carotid plaque composition using three groups of simultaneous non-contrast angiography and intraplaque hemorrhage(SNAP)sequences in 3.0T magnetic resonance imaging(MRI)To study the best display sequence of plaque components;provide a reliable basis for clinical treatment.Methods:Fifty-two patients with plaque formation diagnosed by carotid ultrasound were collected and bilateral carotid 3.0T MRI examinations were performed,including T1-Weighted Imaging(T1-Weighted Imaging)and T2-Weighted(T2-Weighted)Imaging(T2WI),3D time-of-flight MR angiography(3D-TOF)and SNAP sequences,SNAP sequences are divided into three groups of images,which are bright blood images(inversion recovery,IR Image),proton density weighted effect bright blood image(reference acquisition,Ref image)and the Ref image as the background reference,and obtained the black blood vessel wall image(correct real,CR image)after phase-sensitive recombination inversion phase,MRI takes 82 layers For the image,the above-mentioned sequence scan range and field of view are consistent,and the sequence levels match well.For specific values,see Table 3.1.Comparative analysis of T1 WI,T2WI,3D-TOF,and SNAP sequences showing the consistency of intraplaque hemorrhage(IPH),calcification(CA),and lipid composition.The area displayed in the three sets of images is analyzed to see if the different components show differences in the three sets of Ref,CR,and IR images,and then the carotid plaque is quantitatively measured.The measured value includes the largest tube at the level of the plaque.Wall thickness,minimum wall thickness,average wall thickness,lumen area,vessel area,wall area,wall normalization index(NWI),calcified area within the plaque,and bleeding area within the plaque.Use IBM SPSS 19 statistical software for data analysis.Results:1.The diagnosis of plaque by ultrasound and MRI,the diagnosis of carotid plaque by ultrasound and MRI has no statistical difference(P> 0.05)2.SNAP sequence and traditional multi-sequence diagnosis of plaque components.The consistency coefficients of SNAP sequence and traditional multi-sequence diagnosis of calcification,lipid,and plaque bleeding components were 0.776,0.803,and 0.499,respectively.3.The diagnosis of plaque components in the three groups of SNAP sequence images.The plaque bleeding and calcification components showed statistical differences in the Ref,CR,and IR groups of SNAP sequence images(P <0.05);The lipid components showed no statistical difference in the Ref,CR,and IR images in the SNAP sequence(P> 0.05).4.The relationship between CA and IPH in the plaque and the measured value.The correlation between CA,IPH and the maximum wall thickness in the plaque where the carotid plaque is located is(r = 0.861,P = 0.000),(r = 0.799,P = 0.000).5.The relationship between the lipids in the plaque and the measured values.The correlation between the lipids in the plaque where the carotid plaque is located and the blood vessel area is(r = 0.874,P = 0.000),the lipids in the plaque and the wall The correlation of area is(r = 0.856,P = 0.000).Conclusion:1.In the diagnosis of carotid plaque,there is no difference between ultrasound and MRI2.Among the diagnostic plaque components,the SNAP sequence has good consistency with the traditional multiple comparison sequences in identifying the plaque CA,IPH,and lipid components.It has obvious advantages in detecting IPH.3.Among the diagnostic plaque components,the CA within the plaque in the SNAP sequence showed the best image in the Ref group;the IPH showed the best image in the CR group;the Ref,CR,and IR group images showed no effect on the lipid component.difference.4.CA and IPH in the plaque are correlated with the measured values.Both CA and IPH in the plaque where the carotid plaque is located are correlated with the maximum wall thickness,positively correlated.5.The lipid in the plaque is correlated with the measured value.The lipid in the plaque where the carotid plaque is located is correlated with the area of the blood vessel;the lipid in the plaque is correlated with the area of the tube wall,positively correlated.
