Objective:Coronary artery disease?CAD?is the main factor threatening the health of elderly people,how to improve the bad prognosis of cardiovascular disease is an urgent problem in the prevention and treatment of coronary heart disease.The reduces of skeletal muscle mass?SMM?increases the risk of cardiovascular disease in elderly people,we suggesting that Skeletal muscle mass and cardiovascular disease may share a common pathophysiological process.However,the mechanism of skeletal muscle mass affecting cardiovascular development has not been reported at home and abroad.The purpose of this study was to investigate the relationship between SMI?VO2 max and poor cardiovascular prognosis in patients with Stable coronary artery disease?SCAD?at 2 years.Methods:165 inpatients who were diagnosed with stable coronary heart disease by coronary angiography or coronary artery CTA in the Department of Geriatrics,Second Affiliated Hospital of Chongqing Medical University were enrolled from March 2017 to December 2017.Collected general information such as gender,age,height,weight,BMI,smoking and alcohol history.All patients participated in the cardiopulmonary exercise test,and the maximum oxygen uptake was used as the evaluation of aerobic capacity.SMI was calculated by bioelectrical impedance analyzer.The patients were followed up for a total of 2 years through the outpatient medical record system and telephone follow-up,the end points of follow-up were MACCE.Cox proportional hazards model was used to estimate hazard ratio,ROC measured the predictive value of SMI and VO2 max for MACCE.Results:a total of 154 patients were analyzed in this study.During the follow-up period,MACCE occurred in 16 patients?10.4%?.Multivariable Cox regression analysis identified that SMI and VO2max were independently correlated with the prognostic risk of stable coronary heart disease?P<0.05 respectively?.The predictive effect of SMI combined with VO2 max on MACCE was stronger than SMI and VO2 max alone?P<0.05?.At the same time,linear regression suggested that there was a positive correlation between SMI and VO2 max in elderly SCAD patients?P<0.05?.Conclusion:SMI and VO2 max may be independent predictors of poor cardiovascular prognosis in elderly SCAD patients at 2 years,SMI combined with VO2 max can enhance the predictive value of MACCE risks.Early muscle strength training can enhance skeletal muscle mass,improve cardiopulmonary endurance,and improve the long-term prognosis of elderly SCAD patients.
