Objiective:To explore the vestibular dysfunction of patients with idiopathic sudden hearing loss in Qinghai,in order to provide more theoretical support for the clinical diagnosis and treatment of idiopathic sudden hearing loss.Methods:One hundred and eighty-four patients with idiopathic sudden hearing loss who met the inclusion criteria in Qing Hai Hospital from December 2017 to December 2019 were selected and divided into two groups according to whether they were accompanied by vertigo or not.The abnormal conditions of vestibular caloric test,o VEMP,c VEMP and positional test between the two groups were analyzed,and the vestibular dysfunction was analyzed.At the same time,the distribution of abnormal vestibular caloric test,o VEMP,c VEMP and positional test in different types of hearing loss of idiopathic sudden hearing loss were compared.Results:In this study,there were 184 patients with idiopathic sudden hearing loss,including 73 patients with vertigo(39.7%,73/184).The positive rate of vestibular caloric test in idiopathic sudden hearing loss with vertigo group was 67.1%.The positive rates of c VEMP and o VEMP were 54.8% and 69.9%,respectively.Compared with the group without vertigo group,it was higher,showing a statistically significant difference,and mainly saccule impairment.Among the different types of hearing loss in patients with idiopathic sudden hearing loss,the positive rate of caloric test in total deafness group was significantly different from that in flat decrease group and high frequency decrease group.The positive rate of c VEMP in the total deafness group was significantly higher than that in the flat decrease group and the high frequency decrease group,and the difference was statistically significant.The positive rate of o VEMP in the total deafness group was different from that in other groups.Among the patients with idiopathic sudden hearing loss,31 cases were associated with BPPV(16.8%,31/184),most of them had hearing loss above 60 d B,including 17 cases of posterior semicircular canal,9 cases of external semicircular canal and 5 cases of multiple canal(posterior semicircular canal + external semicircular canal),and the latter semicircular canal accounted for the most.Conclusion:1.The rate of vestibular dysfunction in patients with idiopathic sudden hearing loss with vertigo group was significantly higher than that in patients without vertigo group,and the damage of vestibular function in patients with vertigo was mainly saccule impairment.2.Among the types of hearing loss of idiopathic sudden hearing loss,the incidence of vestibular dysfunction was the highest in the total deafness group.3.Among the patients with idiopathic sudden hearing loss,16.8% were associated with BPPV(31/184),followed by external semicircular canal and multi-canal(external semicircular canal + posterior semicircular canal).The more severe the hearing loss,the more severe the damage of vestibular function.
