| Objective: Under the background of medical price reform in public hospitals,this study analyzes the medical expenses and cost structure of medical insurance patients in a city in Hebei province.Analyze the changes in the medical costs and structure of medical insurance for urban employees before and after the reform of medical prices in public hospitals from the perspective of outpatient(emergency)and inpatients,implement policies and measures for medical price reforms in public hospitals for public hospitals in Hebei Province,and improve medical cost control mechanisms,To further improve the efficiency of hospital management to provide a reference.Methods: Collected the medical prices of urban employee medical insurance patients before and after the reform(2016.08.26-2017.08.25)and after the reform(2017.08.26-2018.08.25)in a public hospital in a city in Hebei Province.All the cost data are compared using the comparative research method,with the non-participating hospitals as a control,comparing the changes in the outpatient(emergency)expenses,inpatient expenses,and cost structure before and after the implementation of the medical price reform policy of the public hospitals.The effect of related policies in hospital reform on controlling patient medical expenses.Results:(1)After the medical reform,the average cost of outpatient(emergency)consultations of urban employee medical insurance patients in participating hospitals decreased significantly,with a decrease of 42.83%.Among them,other costs,material costs,western medicine costs,and traditional Chinese medicine costs fell sharply.,But with less impact on average cost per second.After the medical reform,the cost structure of outpatient(emergency)consultations in participating hospitals has been optimized to a certain extent.Western medicine fees,traditional Chinese medicine fees,inspection fees,and laboratory fees are still the main components.However,the total proportion of drug fees has decreased.The proportion of consultation fees has increased.(2)After the medical reform,the average cost of hospitalization for urban employee medical insurance patients in participating hospitals increased slightly,an increase of 3.15%.Among them,the cost of traditional Chinese medicine,western medicine,materials,and other expenses showed a significant downward trend,and the total decline in drug costs was the largest;Investigation,nursing,treatment,and surgery costs have increased.After the medical reform,the structure of hospitalization expenses of participating hospitals has been optimized to a certain extent.Although Western medicine costs,traditional Chinese medicine costs,inspection costs,and laboratory fees are still the main components,the total proportion of drug costs and the proportion of materials costs have decreased significantly.The proportion of medical expenses and nursing expenses has increased.After the medical reform,the out-of-pocket expenses of the urban employee medical insurance patients in the participating reform hospitals increased slightly,and the reimbursement ratio also increased,an increase of 0.49%.Conclusion: After the pharmaceutical price reform in public hospitals,the cancellation of the drug markup policy was fully implemented,which reduced the total proportion of drug costs in the medical costs of medical insurance patients for urban employees in participating hospitals and optimized the cost structure.After the medical reform,the average cost of outpatient(emergency)consultations in participating hospitals decreased significantly,and the average cost of inpatients increased slightly.It can be seen that the policy has insufficient control over the total amount of inpatient expenses,and the increase in controllable expenses such as laboratory fees and inspection fees is still relatively small Big.Public hospitals in Hebei need to further deepen the reform of the medical system,standardize the pricing mechanism for public hospitals’ medicines,advance the pricing mechanism for medical services in public hospitals,improve the government compensation mechanism,formulate a reasonable medical insurance policy,and establish a more comprehensive hospital management system centered on patients.Further reduce the burden of patient medical expenses. |
