Objective:The prolonged survival time of patients with chronic heart failure brings serious harm to the family and society,and it is easy to cause response transfer in the evaluation of the effect of treatment.Existing studies rarely focus on response transfer,and only perform classification analysis based on admission.In response to this problem,this topic uses the method of potential metastasis analysis to identify different subtypes of chronic heart failure patients and the changes in subtypes in longitudinal time,to understand the disease development trajectory of patients with different subtypes,and to identify the patient's response to metastasis.Individual entry level analysis of the migration characteristics of the patient's PRO provides an effective basis for clinical diagnosis and treatment.At the same time,according to the rationality of the results,the report outcome scale of chronic heart failure patients can be further optimized to provide a new measurement tool for the reasonable choice of diagnosis and treatment of clinical chronic heart failure patients,evaluation of treatment effects,and identification of changes in patient healthMethods:This study collected the Chronic Heart Failure Patient Reporting Outcome Scale(CHF-PROM)manual completed from May 3,2017 to December 31,2019,inpatients diagnosed with chronic heart failure from three hospitals in Shanxi Province.Follow-up was performed one and six months after discharge.Then use the potential metastasis analysis to analyze the scale scores at three time points,use the dimensions of each dimension as the explicit variables,establish the potential metastasis model according to the field,and use the maximum likelihood method to estimate the model parameters,and finally get the three time points for each patient.Potential categories,and analyze the impact of demographic indicators and interventions on potential category transfersResults:In this study,700 baseline questionnaires and 530 follow-up questionnaires were finally issued,and 550 baseline questionnaires,504 follow-up questionnaires and 497 valid questionnaires were recovered.In this study,there were 280 male patients and 217 female patients,with an average age of 67.39±14.39 years.Patients were divided into high-level and low-level groups in four areas based on the analysis of potential metastases.According to the two follow-up visits,the patients' metastatic trajectories in the high-level group and the low-level group were obtained.In addition,the study also obtained the influencing factors that affect the state transition in the four fields.The physiological fields are gender,age,tea,smoking,drinking,physical exercise,and light diet;the psychological fields are gender,occupation,smoking,drinking,and physical exercise;the social field is age;the therapeutic field is education and incomeConclusion:This study was based on the scores of the reported outcome scale for patients with chronic heart failure,and was classified in different fields at three time points.The four fields were divided into high-level and low-level groups,and the state transition between the three time points was studied..Therefore,the quality of life of patients at different time periods can be more accurately studied,and the biases caused by the patients' reflection can be avoided.In addition,the influencing factors of state transition in various fields are analyzed,which can provide more suitable programs for the treatment of different types of patients,thereby improving the quality of life of patients and reducing the readmission rate and mortality.According to the changes of the groups before and after the patients,appropriate intervention measures can be given to patients who may develop to a low level,and finally"early prevention,early diagnosis,early treatment" can be better achieved,and the mortality and rehospitalization rate of patients with chronic heart failure can be reduced.
