| Objective:Chronic heart failure is one of very important at this stage of cardiovascular disease,most patients with heart disease and other major chronic disease care is provided by family nursing personnel,a number of studies have shown that take care of chronic heart failure family caregivers burden has become one of the more common problems and the need for attention,therefore,to help the family caregivers found dealing with chronic heart failure care burden of the strategy is particularly important,this study was to explore and determine the needs of family caregivers chronic heart failure;To construct an intervention program for care skills for family caregivers of chronic heart failure according to their needs and verify the effect of the intervention program on patients and caregivers of chronic heart failure.Methods:1.September 2018-November 2018,qualitative research phase: select representative caregivers for qualitative research.In-depth interviews were conducted with the primary caregivers of 12 patients with chronic heart failure to analyze the problems and mood experience of the caregivers during their care.Researchers and experts developed transitional nursing interventions based on the results of qualitative studies.2.Implementation of transitional nursing mode from December 2018 to July2019: randomized controlled trial was used.86 cases of chronic heart failure patients and family caregivers were selected from the heart disease ward of a tertiary a hospital in taiyuan.The caregivers were randomly divided into the control group and the intervention group,each with 43 cases.The control group received routine nursing,and the intervention group received transitional nursing services on the basis ofroutine nursing.All data were analyzed and processed with SPSS22.0 statistical software to evaluate the implementation effect.Results:1.Qualitative research stage: after in-depth discussion of the care experience of family caregivers with chronic heart failure during the transition period,the following common problems were found among caregivers during the transition period: lack of professional knowledge of disease care;Misconceptions about caring skills;Inability to use resources effectively;Interpersonal and psychological counseling barriers.2.Implementation phase of transitional nursing mode: 86 patients with chronic heart failure and family caregivers were randomly divided into the intervention group and the control group,with 43 cases in the intervention group and the control group.There was no statistical difference in the general data of patients and family caregivers.Intervention results: after intervention,the score of family caregiver in the experimental group was lower than that in the control group,and the score of social support was higher than that in the control group,the difference was statistically significant(P < 0.05).Conclusion:The results show that transitional nursing mode can be applied to family caregivers of patients with chronic heart failure,and can effectively reduce the burden of family caregivers of patients with chronic heart failure,and improve their social support. |
